The Pool Fitness Center and Gymnasium close 15 minutes before the facility closing time.

MEMBERSHIP & ACCESS

Membership cards are issued to all members. Each member must swipe
their card upon entering and exiting the facility. Access to the center will be
denied if membership card is not presented and/or membership fees are
not current. Replacement cards are $5 each.

Children under 12 years of age must be supervised by a parent or guardian

at all times when on the premises.

Use of the Fitness Center is limited to those ages 16 and up.

NOTE: The administration reserves the right to suspend and/or cancel

membership of individuals not adhering to Recreation Center rules and
regulations. Throughout the year, photographs are taken during activities,
programs and classes. These photographs may be used for publicity
and/or in promotional materials for the Center. We reserve the right to use
all photographs.

It is understood that this agreement is for twelve (12) months at the Secaucus Recreation
Center and that this agreement will be terminated after twelve (12) months unless my
membership is renewed within thirty (30) days prior to its termination. Membership may be
cancelled prior to the twelve (12) month commitment ONLY with written medical proof of
disability or serious injury, or proof of relocation to a new permanent location beyond a twenty-
five (25) mile radius. It is also understood that the Secaucus Recreation Center may revoke my
membership for violation of the Center's rules and regulations by myself, members of my family
or my guests. It is understood that I am required to return all membership cards to the Secaucus
Recreation Center upon termination of my membership. Initials: ___

A. Members are expected to pay their membership fees in full at the time of application
unless this is a month by month membership. It is understood that any membership that is
payable monthly is based upon a monthly membership fee. I further understand that if my credit
card is denied for ANY reason I will be prohibited from using the Secaucus Recreation Center
until I provide the Secaucus Recreation Center with a new, valid credit card to charge my

B. The Secaucus Recreation Center permits members to bring guests to the facility. Each
adult member (18+ yrs) may bring one guest per visit, however each guest is limited to (5) visits
annually. Guests must be accompanied by a member and are entitled to the same privileges as
the member. All guests are required to sign in at the reception desk. A $10.00 guest fee will be
charged per visit.

C. Children under the age of twelve (12) must be accompanied by an adult at all times
while using the Secaucus Recreation Center.

D. The Secaucus Recreation Center and its staff shall not be held responsible for lost, stolen
or damaged belongings. The lost and found is located behind the reception desk.

E. The Secaucus Recreation Center reserves the right to change or alter facility hours,
programs and classes as necessary.
F. Each member will be issued a membership card. All members are required to have a
photo taken at the Center for identification purposes. The membership card must be presented at
the reception desk upon entering the facility. Membership cards may not be used by anyone other
than the Member. There is a $5.00 charge for replacement membership cards.

G. Secaucus Recreation Center memberships are non-transferable.

H. Members are required to provide their own locks for temporary use in the men's and
women's locker rooms. Any locks not removed at the end of the day will be cut off the lockers
by the staff.

I. The Secaucus Recreation Center includes use of the following services as part of the
annual membership: use of the pool, fitness center, fitness track, group exercise classes, aquatic
exercise classes and use of the locker rooms. Supplemental services may be at an additional cost
with discounts to members. Members understand that during some portions of the year, because
of the use by Recreation/Bd. of Education and/or supplemental services, portions of the facility
may be temporarily unavailable. Additionally, the Town of Secaucus reserves the right to close
the center because of inclement weather and/or any condition that would make the Center, or any
part thereof, dangerous to health and welfare to members and/or
staff.

J. All members are required to read and sign an Agreement and Release of Liability.

The Member, upon execution of this agreement, hereby purchases a membership to the
Secaucus Recreation Center and is subject to the terms set forth herein.

AGREEMENT AND RELEASE OF LIABILITY

1. In consideration of gaining membership or being allowed to participate in the activities and
programs of the Secaucus Recreation Center and to use its facilities, equipment and
machinery in addition to the payment of any fee or charge I do hereby waive, release and
forever discharge the Town of Secaucus and its officers, agents, employees, representatives,
executors and all others from any and all responsibilities or liability for injuries or damages
resulting from my participation in any activities or my use equipment or machinery in the
above-mentioned facilities or arising out of participation in any activities at said facility. I do
also hereby release all of those mentioned and any others acting upon their behalf from any
responsibilities or liability for any injury or damage to myself or to my family members,
including those caused by the negligent act or omission of any of those mentioned or others
acting upon their behalf or in any way arising out of or connected with my participation in
any activities at the Secaucus Recreation Center or use of any equipment at the Secaucus

2. I understand and am aware that strength and aerobic exercise, including the use of
equipment, is a potentially hazardous activity. I also understand that fitness activities involve
a risk or serious injury and that I am voluntarily participating in these activities and using
equipment and machinery with knowledge of the risks involved. I herby agree to expressly
assume and accept any and all risks of serious injury. (Initials: )

3. I do hereby further declare myself to be physically sound and suffering from no condition,
impairment, disease, infirmity or other illness that would prevent my participation in any
activities and programs of the Secaucus Recreation Center or use of equipment or machinery
except as hereinafter stated. I do hereby acknowledge that I have been advised of the need for
a physician's approval for my participation in an exercise/fitness activity or in the use of
exercise equipment and machinery. I also acknowledge that it has been recommended that I
have a yearly or more frequent physical examination and consultation with my physician as
to physical activity, exercise and the use of exercise and training equipment so that I might
have recommendations concerning these fitness activities and equipment use. I acknowledge
that I have either had a physical examination and have been given a physician's permission to
participate, or that I have decided to participate in activity and or/use equipment and
machinery without the approval of my physician and do hereby assume all responsibility for
my participation and activities, and utilization of equipment and machinery in my activities.

4. This Agreement and Release of Liability shall remain in effect for my initial membership and
all subsequent renewals of membership, and/or my participation in any and all activities and
programs of the Secaucus Recreation Center, or use of it facilities, equipment and machinery,
presently or in the future, without the need for the entry into a new Agreement and Release of
