2010 Needs Survey Questionnaire and Information Request

The following questions require input from District and/or School Personnel. Please review the questions and have
the information available for Assessors at the time of the site visit.

District Questions

In order to expedite the survey process and gather accurate and complete information for each District, the
Superintendent or a designee should be prepared to answer these questions when Assessors arrive in the District for
site visits:

1. Does the District have a Master Plan? If Yes, please provide the following information:
• Year Completed;
• Name of Firm that prepared the Master Plan;
• Is a summary available?
• Has summary been provided to the MSBA?

2. Are there any District-controlled buildings that were previously used as educational space and which are no
longer used as educational space? If yes, please provide the following information for each building that
is no longer used as educational space:
• Building Name and address;
• Year opened;
• Last year used as school;
• Current use;
• Reason the building ceased to be used as educational space:
o Operating costs;
o Lack of Enrollment;
o Building Condition; and/or
o Other.
 If Other, please provide brief description.

3. Does the District have any shared campuses? If yes, please answer the following questions:
a. Are there multiple school buildings on one campus?
b. Are there multiple schools within one building?
c. Please provide the names of all schools which share a campus.

School Questions
In order to expedite the survey process and gather accurate and complete information for each school, the School
Principal, Facilities Director, and/or a designee with knowledge of the history and use of the school should be
prepared to provide the following information during the on-site assessment:

1. Design Enrollment;
2. School Area in Gross Square Feet;
a. If the school is composed of multiple buildings, please provide the combined Gross Square
Footage.
3. Grades Served;
4. Year Opened;
5. Whether the school has been designated as Historic;
6. Total Permanent Staff (FTE);
7. Number of interior classrooms that lack views to the outside;
8. Whether the school has internet access;
a. If Yes, is it wireless?
b. If there is wireless internet access, it is full or partial?
9. Whether classrooms have at least one computer for instructional use;
a. All Classrooms
b. Most Classrooms
c. Some Classrooms
d. No Classrooms
10. Presence and extent of Surveillance Camera Coverage;
11. Presence and extent of Intrusion Alarms;
12. Presence and extent of Metal Detector Coverage;
13. Whether there is a Secure Entry;
14. Number of Full Time Security Staff, if any;
15. Number of elevators, if any;
16. Whether there is an imminent danger of structural failure that could cause significant harm to building or
occupants? If an imminent danger of structural failure exists, please answer the following questions:
a. Has an engineering professional provided a written opinion regarding the structural deficiency?
b. If Yes, provide the Name of the Firm and Date of Report.
17. Special Education Delivery;
• Total Inclusion: Yes/No
• Partial Inclusion: Yes/No
• Pull-Out/Tutorial: Yes/No
• Self-Contained: Yes/No
• Collaborative: Yes/No

19. General Information about Site Conditions such as Septic Systems, Sewer, Well, Hazardous Materials
and Wetlands
20. Buildings: For each building at the school, be prepared to provide the following information:
• Year Built;
• For what grade level the building was originally built;
• Number of Floors;
• Presence and extent of Air Conditioning, if any;
• Whether the building is occupied or vacant;
• Be prepared to provide a history of all Additions, Renovations and installations of Modular
Classrooms at each school building, including:
o Year of Addition, Renovation, or Modulars;
o If Modulars were installed, please provide the Number of Modulars, Age of Modulars, and
number of Modular Classrooms;
o Brief Description of addition/renovations, (e.g. Addition of 10 classrooms in 1998 or all
single-paned windows replaced in 2000.)
21. Building Systems:
• Roof
o Age of oldest roof section in years;
o Type(s) of roofing material;
o Total Square Footage of the roof;
o Year of last roof repair or replacement;
o Brief Description of last repair or replacement
• Exterior Windows
o Age of oldest windows in years;
o Type(s) of windows;
o Year of last window repair or replacement;
o Description of last repair or replacement
• Boilers
o Age of oldest boiler in years;
o Year of last boiler repair or replacement;
o Description of last repair or replacement;
• HVAC
o Age of oldest components in years;
o Year of last HVAC repair or replacement;
o Description of last repair or replacement;

A. In addition to collecting the information above, the Assessors will record room usage in the school during their
site visit. To facilitate this, please have a floor plan of the building that clearly shows each room in the school.
A fire egress plan works well for this purpose. Using the floor plan School Personnel shall designate what each
room is used for. We understand and appreciate the value of the School personnel’s time, so we ask that this
floor plan be filled out prior to the site visit. The Assessor will then review it with the School representative at
the time of the assessment and collect a copy of the floor plan for their records. The following are types of
rooms that will be counted and some information that will be needed.

1. Classroom types.
• General Classrooms;
• Science Labs;
• Science Classrooms;
• Computer Labs;
• Special Education Classrooms;
• Other Special Education Classrooms (e.g. Resource Rooms, OT/PT Rooms);
• Art;
• Music;
• Vocational Classrooms;
• Vocational Shops;
• Non-Traditional Classrooms (e.g. large closets, hallways, or stages that are being used for
educational space);
• Collaborative Spaces
o Please provide the number of classrooms, the name of the Collaborative, and the
approximate Square Footage used by the Collaborative.
• The Number of Classrooms that are not used as classrooms, including:
o District Administration Offices;
o Storage Rooms;
o Other
2. Library/Media Center
a. If the School doesn’t have a Library/Media Center, does it share one with another school?
3. Cafeteria
a. How many lunch seatings?
b. If the school does not have a cafeteria, does it share one with another school?
4. Gymnasium:
a. How many gyms?
b. How many full-size basketball courts?
c. If the school does not have a gymnasium, does it share one with another school?
5. Does the school have a Multipurpose Room that is used as a gymnasium?
6. Auditorium
a. If the school has an auditorium, what is the seating capacity?
