-Ask directly; have you ever thought about hurting
self and do you have a plan?
-Initiate safety protocols, removing anything that will
inflict self-injury throughout the entire stay at the
facility.
-Use warm, caring, nonjudgmental manners until
discharge.

Patient will verbalize desire
to work with father towards
heathy communication
skills. Needs time ref

Around peers seems happy, outgoing, loud, able to be self, makes eye
contact.
Looses all sense of happiness when his father is brought up in
conversation, which gets worse when father is present.
Body language: tense, little to no eye contact.
Even at his lowest point he is unable to speak to father about future
plans/goals for self/passions. Extreme pressure to excel in school with
grades and become what his father wants him to be. Communication is
one way, fathers way, feeling of hopelessness from son is observed.
Try reviewing the mental status exam posted in Blackboard to be
sure you havent omitted anything and to help organize this
section. You have noted the change to his mood, affect, eye
contact, and sense of hope when he is with his father versus his
friends well. Good job

Evaluation
Patient will not harm self in hospital. This is a
goal, not an evaluation. And, you still need to
evaluate the goal you set. Did Neal identify
triggers? You can use this as another outcome
goal and then evaluate that here as well.
