Each group member will complete this confidential form (only you and your instructor will see it).
The completed form will be submitted to the Dropbox by each group member individually no
later than Sunday 11:59 PM EST/EDT of Module 4. If a group members participation is
minimal, 25 percentage points will be deducted from the group members grade. If a group
member fails to participate on the assignment, a grade of zero will be assigned.
