VASCULAR ACCESS CARE
Temporary Access Site (Catheter inserted) 1. Intrajugular 2. Subclavian 3. Femoral Guidelines: 1. Once the temporary catheter site is inserted, the patient/relative must update the nurse to record the following information: date of the operation name of the vascular surgeon name of the hospital/institution the operation is made 2. The patient/relative must assess and report to the nurse on duty the following sign and symptoms: any bleeding/ soaking of the blood in the
dressing and pain on the insertion site.

3. The patient must keep the dressing clean and dry.
Swimming is not allowed and taking a bath must be assisted with a relative so as to avoid moisture in the insertion site that causes infection.
4. Wear a mask over your nose and mouth anytime the
catheter is opened to prevent bacteria from entering the catheter and your bloodstream. Professionals changing
the dressing should wear a mask and gloves as well.
Must make sure that the anti-bacterial ointment is applied on the insertion site after catheter care.
5. The caps and the clamps of your catheter should be
kept tightly closed when not being used for dialysis. Only your care team should use your dialysis catheter to draw blood or to give medications or fluids. 6. Avoid physical activity that might dislodge the access, which could result in excessive bleeding and air entering the circulatory system. 7. If the catheter exit site bleeds, apply firm pressure over the bleeding site with a clean gauze. Proceed immediately to the hospital and inform the dialysis staff immediately.

2. The patient/relative must assess and report to the

3. The patient must not allow to take blood pressure and
blood extraction on the affected limb. 4. The patient must avoid tight straps, watchbands, jewelry or clothes that will restrict the limb. 5. Avoid sleeping on the affected limb. 6. Do not use creamsor lotions over the access site.
Permanent Access Site
1. AVF or Arteriovenous Fistula 2. Gortex/Graft Guidelines: 1. Once the permanent site is made, the patient/relative must update the nurse to record the following information: date of the operation name of the vascular surgeon
7. If the access site bleeds, apply firm pressure over the
bleeding site with a clean gauze until it stops. Proceed to the hospital and inform the dialysis staff immediately if bleeding does not stop within 30 minutes of firm pressure. 8. Dont carry any heavy objects or lift heavy objects greater than 5 pounds with the affected limb. 9. If the patient has any pain in the affected hand or fingers including paleness, coolness, numbness,
