PETITION FOR ADOPTION OF MINOR

MATTHEW ANGELO JAREO
Petitioners, Spouses Ray Gerard and Annie Ramos, by counsel,
most respectfully state:

1.

Petitioners are husband and wife, both of legal age, Filipino

citizens, and residents of 2280-A Arellano St., Sta. Ana, Manila;

CABUTIN (Minor Child), who was born to Evelyn B. Cabutin on 06 April

2002, at the Amang Rodriguez Medical Center, Marikina City. A certified
true copy of her Birth Certificate of Live Birth is attached as Annex A;

The minor child was voluntarily and irrevocably surrendered

by her biological mother, Evelyn B. Cabutin, to the Department of Social

Welfare and Development-Natioal Capital Region (DSWD-NCR) through
the Heart of Mary Villa, a non-stock, non-profit child caring agency duly
licensed and accredited by the DSWD. The Deed of Voluntary
Commitment dated 23 April 2002 is attached as Annex B;

This petition for adoption is also an application for the

change of name of the minor child from BETTINA ADRIELLE CABUTIN to

ERIKA MARIE GUECO RAMOS, following the surname of her adoptive
parents, herein petitioners;

Petitioners are qualified to adopt the minor child. They are in

possession of full civil capacity and legal rights;

6.

They are of good moral character and have never been

convicted of any crime involving moral turpitude. Attached as Annexes

C and D are copies of their respective clearances issued by the
National Bureau of Investigation (NBI);

7.

The petitioners are emotionally and financially prepared to

raise the minor child in a healthy and loving environment. A copy of their
financial statement for the year 2003 is attached as Annex E;

and have satisfied

all the

requirements prescribed by the DSWD. The minor child was matched to

the petitioners, who were approved as adoptive parents, by the DSWDNCR and the Kaisahang Buhay Foundation (KBF), a non-stock, nonprofit child and family welfare agency duly licensed and accredited by the
DSWD-NCR. Attached, as Annex F is the Affidavit of Consent to the
adoption issued by the DSWD-NCR through its director, Alicia R. Bala.
The Pre-Adoption Placement Authority likewise issued by the DSWDNCR is attached as Annex G;

The minor child was subsequently given to the petitioners on

supervised trial custody. During the trial custody period, a licensed

social worker from the KBF conducted home visits to ensure that the
minor child was being cared for properly. The social worker, in her Social

Case Study Report (Home and Child Study Report) (Annex H),
recommended that the adoption of the minor child by the petitioners be
granted. She concluded that the minor childs adoption by petitioners is
in her best interest.

WHEREFORE, it is most respectfully prayed that after due hearing,

this petition be granted, and that 1) the minor child BETTINA ADRIELLE
CABUTIN be declared, for all intents and purposes, as the child of the
petitioners, and 2) her name be changed to ERIKA MARIE GUECO
RAMOS, following the name of her adoptive parents.

age, after having been duly sworn to in accordance with law, depose and
state the following:
1. We are the petitioners in the above-captioned case;
2. We caused the preparation of this Petition for Adoption of
Minor Bettina Adrielle Cabutin;

3. We have read the contents thereof and we certify that the facts
stated therein are true and correct based on our personal
knowledge and on available records;
4. We have not commenced any other action or proceeding
involving the same issues in any Court or in any other tribunal
or agency;
5. To the best of our knowledge and belief, no such action or
proceeding has been filed or is pending in any Court or any
other tribunal or agency.
