Introduction
Case studies allow us to become personally invested in the care of another through a
scenario. According to Casotti, Beneski, & Knabb, (2013), case studies tell real life stories and
ask for us to use our knowledge to problem solve. Using clinical reasoning, students will be able
to assist with solving problems. Clinical reasoning requires students to utilize the nursing
process by gathering their information, synthesizing the information, come to a reasoned
judgment, and implement interventions to promote health (Bastable, 2014). Assessment of
interventions, although not listed for this assignment would be verified by follow up blood lead
level decreases in the child in our case study.
Case Study and Questionns
Each student will be provided with the following online case study and five questions.
Internet access will be provided for students that wish to access the online sites that we have
been discussing in class. After reading the case study, answer each question on the fillable
question form print it, and turn it into the drop box in the front of the class. When answering the
questions, I am most concerned with the depth and breadth of the answers that you provide. No
credit will be given to students that do not complete the test.
You have been assigned the Madison family. Dad (Sam) is 25 years old and works the
night shift full time at Republic Steel. Mom (Audrey) is 25 years old and is a teacher with the
Buffalo Public Schools. They have one child, Amanda who was found to have an elevated blood
lead level at her two year well child checkup; 15 mcg/dL.. They live at 418 Grant Street,
Buffalo, NY 14213. They purchased and moved into this residence 11 months ago, when Amada
was 13 months old. Amandas blood lead level at her one year checkup was < 1 mcg/dL. Prior

to them buying this home and moving in, they lived in a newly constructed condominium at
3818 Powers Road, Orchard Park, NY 14127.
1. Identify internet resources that you will utilize prior to meeting this family and discuss
why. Discuss what information you will be looking for and why.
2. Are there questions that you will have for dad (Sam) or mom (Amanda) in regards to
their employment? If you answered yes, please list questions and provide the rationale for
asking them.
3. Once in the home, what will you be looking at and for? Discuss common hazards.
4. When talking with the family about their daughters home habits such as play time,
what questions will you ask them and why?
5. Discuss county agencies that can assist this family. Provide a rationale for each
choice.
Content Description
The purpose of this assignment is to provide the student with clinical experience in the
promotion of wellness and prevention of illness for diverse individual clients and families.
Emphasis is on risk appraisal, environmental assessment, and the context of family in wellness
promotion and risk reduction.
Appropriate Level
Because my class consists of BSN prepared nurses, the content of the case study and test
is appropriate for their educational level. At this stage of their education, critical thinking and
the ability to implement interventions is a must. The focus of this class is to draw out the skills
that these nurses already have and apply them to their everyday work situations.

Apply concepts of wellness promotion and disease, injury, and disability prevention with
case study individuals.

Apply primary, secondary, and tertiary prevention strategies.

Assess environmental factors that influence the healthcare needs and preferences of
individuals as members of families.

Describe how epidemiology and surveillance can be used to safeguard public health.

There are five questions that are required to be answered after the case study is reviewed.
Total number of points possible is 100%. Each question will be weighted at 20%.
