attend the camp, as well as the transportation to and from those sites and participate in all phases of the
COUNCIL WIDE ECOLOGICAL AND SPIRITUAL CAMP PROGRAM held by the Boy Scout of the
Philippines on July 28-31 at Balsahan Pool, Iwahig Penal Farm, Puerto Princesa City. I understand that the staff
will exercise reasonable care to ensure my childs safety.
Adults accompanying the group will provide supervision and will exercise reasonable care to avoid accidents.
By signing below I am granting my child permission to attend and agree to the terms discussed above.
I, as parent and/or legal guardian, remain legally responsible for any personal actions taken by the above
named/participant. And all consequential damage arising from or in connection with my child attending the
event or in connection with any illness or injury or cost of medical treatment in connection therewith.
