DISPATCH NOTES:Carrier must immediately notify Team Eagle Logistics whenever there is a discrepancy in the
shippers instructions, Bill of Lading, or this Carrier Rate Confirmation. Team Eagle Logistics terms for payment are (30)
days from the date of receipt. Please email the invoices to ap@teameaglelogistics.com The rate above includes all fees
associated with this shipment. The Carrier has (24) hours to notify Team Eagle Logistics of additional fees. There will be
no additional fees accepted unless in writing from Team Eagle Logistics. The Driver cannot cut the seal. If there are any
issues the driver must contact Team Eagle Logistics immediately. Proper load temperature is the driver/carriers
responsibility. Driver must verify at the time of pickup that the Bill of Lading matches. Carrier agrees to the terms and
conditions of the Team Eagle Logistics master Broker-Carrier Agreement, and such agreement governs this Rate
Confirmation between the parties even in the event the carrier does not sign this Carrier Rate Confirmation, but provides
the transportation as described herein. In the event of any conflict between the agreements of the carrier rate confirmation,
the agreement shall govern. The carrier agrees to and must comply with all department of transportation and other
governmental regulations when transporting hazard materials. The carrier agrees that it will be the sole carrier for the
entire shipment and the brokering of said load will result in non-payment. By accepting this load the carrier agrees to
depend, hold harmless and indemnify Team Eagle Logistics against any and all claims, fines or any other expenses related
to the Carriers operations under California Air Resource regulations, that any operating Transport Refrigeration Units
(TRUs) are compliant with the California Air Resource (CARB) TRU Airborne Toxic Control Measure (ATCM)
requirements and regulations for the transportation of freight at any point in the state of California.
