LIABILITY WAIVER, ASSUMPTION OF RISK AND AGREEMENT TO HOLD HARMLESS

MUST BE SIGNED

I understand that dog obedience training is not without risk of injury to myself, my family or my
dog. I/we believe the benefits associated with participation in this activity outweigh the risks of
injury, and voluntarily assume these risks to myself, my family, my dog and any other person
participating with me in this activity. I understand that I and my dog are participating at our own
risk, and that Gentle Touch Pet Spa/Lori Kling and Michelle R. Dunn cannot in any way assume any
responsibility for the health or safety of myself, my family or my dog(s).
I hereby waive and release Gentle Touch Pet Spa/Lori Kling and Michelle R. Dunn, from any and all
liability of any nature, for injury, loss, illness, expense or other incident or hardship or damage
which I or any member of my family or my dog may suffer, including specifically, but not limited
to, any injury or damage resulting from the action of any dog, and I expressly assume the risk of
such damage or injury while attending any training sessions or other function, or while on the
training grounds or the surrounding area thereto.
I hereby agree to indemnify and hold harmless Gentle Touch Pet Spa/ Lori Kling and Michelle R.
Dunn from any and all claims for loss or injury to myself, any member of my family, or my dog
while on the grounds or the surrounding area thereto as a result of any action by any dog,
including my own. Furthermore, I/we personally assume all responsibility and liability should my
dog do any damage to other persons, property or other dogs while on the premises of Gentle
Touch Pet Spa.
I/we have read the foregoing Liability Waiver, Assumption of Risk and Agreement to Hold
Harmless, know and understand its contents and sign it at my/our free will.
