2) Remove the Original UPC from the Qualifying Product box.
Shipping labels or copies of UPC Barcodes will not be accepted in place of the Original UPC.
3) Complete all information requested on this form accurately and legibly.
Limit (2) two rebates per product per address, except where prohibited by law.
4) Mail the Original UPC Barcode, a copy of your receipt, invoice or packing list with purchase date circled,
