applicantssurname,nameandcontactnumberwritteninboldlettersontheupperleftcornerof
theenvelope).
The applicant must come personally on the appointment date and must pay the visa fee upon
submissionoftheapplication.
Alwaysinclude2photocopiesofthefilledupapplicationformand1photocopyoftherestofthe
documents.

REQUIREMENTS:

1. DulyaccomplishedSchengenapplicationform.
2. One(1)recentpassportsizephotowithwhitebackground(tobepastedonthe
applicationform)
3. Validpassport(validforaminimum3monthsaftertheplannedtrip).
4. 1photocopyofthedatapageofthevalidpassportandallpreviousvisasand
stamps.
5. Previouspassports(ifapplicable).
6. ForspousesofSpanish/EUnationalsSpanish/EUmarriagecertificate(issued
not more than 6 months ago) recognized by the Government of the
corresponding EU national. Marriages that are not registered/recognized by
the corresponding EU Government will not be processed as an EU spouse
visa. Only marriages recognized/registered by the corresponding EU
Governmentarefreeofcharge.
7. Minors:Thecorrespondingauthorization.
8. Proof of economic means: Bank Certificate, Bank Books, ITR, International
CreditCards.
9. If invited by relative/friend, OFFICIAL INVITATION LETTER issued by
Spanish Police and photocopy of the sponsors passport and/or D.N.I.,
residencecard,etc.
10. Ifemployed:CertificateofEmploymentandauthorizationforleaveofabsence
(originalcopies).
11. If selfemployed: Business Permit, Registration, Company ITR (original and 1
photocopy).
12. FornonFilipinoapplicants,pleasepresentyourACR,ICRandreentrypermit
(and1setofphotocopies).

only itineraries with Spain as main destination (longest stay) will be
