Emergency Contacts
If the campus is unable to contact the parent/guardian, the school is authorized to call the Emergency Contacts as designated on the enrollment
card by the parent/guardian. The parent/guardian is responsible for providing transportation to obtain medical treatment. If a student is transported
by ambulance, the parent/guardian is responsible for the cost. In the event parent / guardian, physicians, or other persons named on this card
cannot be contacted, school officials are hereby authorized to take whatever action is deemed necessary, in their judgment, for the health of the
aforesaid student. Emergency contacts are only authorized access to information regarding the student and authority to transport the student in an
emergency situation occurring at school when the parent/guardian cannot be reached. At any other time, emergency contacts have no rights

Request for Food Allergy Information


Beginning with the 2011-2012 school year, Texas Education Code 25.0022 requires that school districts request that the parent or guardian
disclose a child's food allergies at the time of enrollment in public school. Additional information regarding food allergies, including the
maintenance of records related to a student's food allergies, can be found in the Board policies at FD and FL. The District is required to maintain
confidentiality regarding any food allergy information provided and may only disclose the information to teachers, counselors, nurses, and other
appropriate school personnel within the limitations of the Family Educational Rights and Privacy Act and Board policy FL.
This form is provided to allow a parent/guardian to disclose whether his/her child has a food allergy or severe food allergy that he/she believes
should be disclosed to the District in order to enable the District to take necessary precautions for the child's safety. "Severe food allergy" means a
dangerous or life-threatening reaction of the human body to a food-borne allergen introduced by inhalation, ingestion, or skin contact that requires
immediate medical attention.
In addition, if the child has severe food allergies, the parent is encouraged to contact:
1) The campus nurse; and
2) The Katy lSD Nutrition and Food Service Department, 281-396-6240 (if the child will be purchasing meals in the school)
"Severe food allergy" means a dangerous or life-threatening reaction of the human body to a food-borne allergen introduced by inhalation,
ingestion, or skin contact that requires immediate medical attention.

Section 37.10 Tampering with Governmental Record
(a) A person commits an offense if he:
(1) knowingly makes a false entry in, or false alteration of, a governmental record;
(2) makes, presents, or uses any record, document, or thing with knowledge of its falsity and with intent that it be taken as a genuine
governmental
record; or
(3) intentionally destroys, conceals, removes, or otherwise impairs the verity, legibility, or availability of a governmental record.
(b) An offense under this section is a felony.

Section 25.001(h) Admission
(a) In addition to the penalty provided by Section 37.10, Penal Code, a person who knowingly falsifies information on a form required for
enrollment of a
student in a school district is liable to the district if the student is not eligible for enrollment in the district but is enrolled on the basis of the false
information.
The person is liable, for the period during which the ineligible student is enrolled, for the greater of:
(1) the maximum tuition fee the district may charge under Section 25.038 of this code; or
(2) the amount the district has budgeted for each student as maintenance and operating expenses.
(A person found guilty of falsifying information on an enrollment form may be liable for tuition for a sum in excess of $5,000.)
Discipline Management Plan and Student Code of Conduct 2016-2017

We have reviewed the Falsification of Information statements, the Katy ISD Discipline Management Plan, and Student Code of Conduct (including
the Student Responsible Use Guidelines for Technology on pages 51-53 ). We understand that these publications reflect current federal and Texas
state law, Katy ISD Board policies, Katy ISD elementary and secondary guidelines, and District and campus regulations. Our signatures certify that
we have reviewed the above publications, including the Student Code of Conduct, and that we understand the contents.
We understand that students who violate the Student Code of Conduct shall be subject to disciplinary action, up to and including
referral to law enforcement offi cials.

Free and Reduced Lunch


The Katy ISD Nutrition and Food Service Department provides healthy meals (breakfast and lunch) each day at nominal cost. Your children may
qualify for free or reduced meals.
At the end of this Registration you will have the opportunity to visit the Katy ISD nutrition site and apply for Free and Reduced meals.

You must apply for this program every year. You are not automatically enrolled in the program. You WILL NOT receive a
paper application from your child's campus.

Student Directory Information


Certain information about district students is considered directory information and will be released to anyone who follows the procedures for
requesting the information unless the parent or guardian objects to the release of the directory information about the student. If you do not want
Katy ISD to disclose directory information from your childs education records without prior written consent, you must notify the district in writing
within two weeks of enrollment. Katy ISD has designated the following information as directory information: student name, address, telephone
listing, and photograph, as well as, honors and awards received, dates of school attendance, grade level, and most recent educational institution
attended, participation in officially recognized activities sports, and weight and height of members of athletic teams.
A - All My childs directory information is public information and may be released to the public. It may be released to anyone making a public
information request and may appear in school directories, other school publications, photographs, yearbooks, in a program for a performance
(such as athletics, band, choir, etc.), on a District or campus Web page, and in news releases. My childs directory information may also be used
on social media platforms to provide immediate information about upcoming events, show learning in classrooms, provide strategies for learning,
highlight student work, connect parents to student learning, and to brand our schools. Many times, teachers use social media to share exceptional
educational practices and to connect with other educators from around the world. Some examples of social media networking sites include, but are
not limited to: Facebook, Twitter, YouTube, Instagram, Blogs, and LinkedIn. NOTE: Parents should select this code if no restrictions are to be made
in releasing information in the types of publications listed. This code should not be confused with a secondary parents selection regarding the M
code for military recruiters and institutions of higher education. The A and M codes are treated independently of each other.

making a public information request and may appear in school directories, other school publications, photographs, yearbooks, in a program for a

directories, yearbooks, programs for performances (such as athletics, band, choir, etc.), in news releases, etc. No pictures will be released. High
school yearbooks are sold strictly by the yearbook publisher using directory information to contact students/parents. If this code is chosen, the
student/parent must contact the yearbook sponsor to obtain contact information for the yearbook publisher since the campus will not be able to
release the student information. This code should not be confused with a secondary parents selection regarding the M code for military recruiters
and institutions of higher education. The N and M codes are treated independently of each other.
O Other My childs directory information is confidential; my childs name, address, phone number, and/or picture may not appear in school
directories or on a District or campus Web page and may not be released to the public; however, name and/or picture, height/weight, organizational
membership, awards received may appear in yearbooks, school-confined publications, and in a program for a performance (such as athletics,
band, choir, graduation programs etc.), may be included in professional school group or class photographs, and may be released to officiallydesignated vendors for school-related purposes (i.e. individual and group picture photographers, yearbook publisher, if necessary in identifying
photos, providing order forms, etc). Vendors will be under contract to not use or share student information for anything other than the school-related
purpose. NOTE: Parents should select this code if student information is restricted, but may be included in school-confined publications other than
a student directory and professional school group or class photographs. High school yearbooks are sold strictly by the yearbook publisher using
directory information to contact students/parents. If this code is chosen, the student/parent must contact the yearbook sponsor to obtain contact
information for the yearbook publisher since the campus will not be able to release the student information. This code should not be confused with
a secondary parents selection regarding the M code for military recruiters and institutions of higher education. The O and M codes are treated
independently of other.

Elementary and Secondary Education Act of 1965 (20 USC Section 6301 et seq.) to provide a military recruiter or institution of higher education, on
request, with the name, address, and telephone number of a secondary student unless the parent has advised the district that the parent does not
want the students information disclosed without the parents prior written consent. My childs name, address, and telephone number is
confidential and may not be released to a military recruiter or institution of higher education. NOTE: This code may not be split to allow only military
recruiters or institutions of higher education access but not the other. In addition to the A, N, and O code, secondary parents/guardians should
select this code if they do not want military recruiters or colleges to obtain their childs name, address, and telephone number. This option is
treated separately from the A, N, and O code chosen for directory information purposes.

Statement of Purpose and Intent
Participation in competitive afterschool extracurricular activities and/or parking on campus in Katy Independent School District (herein after referred
to as the District) is a privilege. These students carry a responsibility to themselves, their fellow students, their parents, and their school to set the
highest possible examples of conduct, which includes avoiding the use of illegal drugs, performance-enhancing drugs, and/or alcohol.
Participation
Each student who desires to participate in competitive afterschool extracurricular activities and/or parking permit privileges shall be provided with
written information regarding the Districts random drug testing policy and a Student and Parent/Guardian Consent to Random Drug Testing form,
which shall be read, signed and dated by the student, parent and/or person otherwise in lawful control of the student. The consent requires the
student to provide a urine sample to be tested for illegal drugs, performance enhancing drugs, and/or alcohol when chosen through the random
selection process. No student shall be allowed to practice or participate in any competitive afterschool extracurricular activities and/or
parking permit privileges until the Student and Parent/Guardian Consent to Random Drug Testing form is properly signed and returned.

I, the above-named student, understand after having read the information regarding the Districts random drug testing, that out of care for my health
and safety and that of other students, the District will enforce the rules applying to the use of illegal drugs, performance-enhancing drugs, and/or
alcohol. As a member of one of the groups designated for inclusion in random drug testing, I realize that the personal decision that I make daily in
regard to the consumption/use of illegal drugs, performance-enhancing drugs, and/or alcohol may affect my health and well being as well as the
possible endangerment of those around me and reflect upon the group with which I am associated. If I choose to violate the random drug testing
policy regarding the use of illegal drugs, performance-enhancing drugs, and/or alcohol any time while I am involved in any activity, including inseason or off-season activities, and/or parking permit privileges, I understand upon determination of that violation, I will be subject to restrictions as
outlined in the random drug testing policy.

Internet, Web Tools, and Email Access


Filtered Internet access is provided to KISD students as defined by the Childrens Internet Protection Act (CIPA). Access will be supervised by
District teachers or staff. If your child is under 13 years of age, the Children's Online Privacy Protection Act (COPPA) requires additional parental
permissions for some educational applications accessible over the Internet. All KISD students in grades 6 - 12 will receive a student email
account. If you do not want your student to have access to the Internet, use educational applications accessible over the Internet, or to have a KISD
email address, please contact your student's campus principal with written notice. For more information regarding these tools, see the
'Responsible Use Guidelines' section of the Discipline Management Plan and Student Code of Conduct.

I have read, understand, and will abide by all the guidelines set forth above regarding the use of personal electronic devices in the District. I
understand that violations of these guidelines may result in the loss of District network and/or computer privileges either temporarily or
permanently, as well as, any other disciplinary action in accordance with the Student Code of Conduct.

care and handling of personal electronic devices while at school. My child is clear that the consequences of not upholding his/her responsibilities

Falsification of a document related to school records is a criminal offense under Section 37.10 Penal Code. In addition, Texas Education Code
25.001 (h) specifies that a person who knowingly falsifies information on a form required for enrollment is liable for tuition costs if the student is not
eligible for enrollment in the district.
