preventive measures, in particular

2. Workers were found working at

audible warning sounds, wear reflector
clothing and provision of banksmen along
with usage of sign boards.

proper marking on the pathway and
adequate training to the operator.

4. Oil spillage from forklift was found on


the surface which may cause a slip, trip
or fall injury to the workers such as
head injury, sprain, or fractures.

housekeeping to clean the oil spillage
and supervision.

also training on housekeeping, proper
maintenance of the forklift.

5. Workers found bypassing the


interlocked guards while working on a
bench mounted metal cutting machine
which could cause slicing of fingers and
arms, abrasion on the skin or even
fatality is possible.

Immediate Action: Get interlock guards


in position, use trip bars near the cutters
and provision of personal protective
equipments

Long Term: Provide alternate guards.


Adequate information, instruction, training
and supervision should be provided.

6. Worker was found fire fighting without


any personal protective equipment and
had no access to fire extinguishers.
This may cause skin burn, or even lead
to death of the worker which may also
affect workers around the area.

Immediate Action: Stop the work, isolate


the fire and inform the fire marshal.

protective equipment. Access to fire
extinguishers and provision of fire
detectors with warning sounds. Also
provide training and supervision for
emergency evacuation.

7. Workers were found manual handling


heavy loads which could cause
musculoskeletal

dropping of load on self, abrasion,
stress and fatigue.

such as forklift, trolley, hoists and lifts.

8. Workers who were found working at


the concrete breaker were exposed to
noise which could cause reduced
hearing sensitivity, Temporary ringing in

personal protective equipments like ear
defenders or muffs and ear plugs. Job
rotation is vital along with supervision to
ensure reduced exposure to noise.

9. Drillers were not using personal


protective equipment and were exposed
to the vibration that could cause white
finger, nerve damage, joint damage and
muscle weakening.

10. Observed that the scaffold not tied


adequately to the building and the work
platform is overloaded. This could
cause a collapse of the structure which
may cause fall from height and falling of
objects which leads to fracture, sprain
or even death.

Immediate Action: Use anchor bolts and


through ties to ensure that the scaffold is
tied to the building, usage of rakers and
bracing to stiffen the scaffold frame work.

information should be provided along with
adequate training and supervision. Good
inspection work should be carried out

11. Lighting level in the machinery area


is less than 600 lux and may lead to
temporary eyesight problem, stress or
headache to the workers.

the required work areas, the light levels

be adjusted anytime along with

13. Found electrical cable trails on the


floor with open wire ends which leads to
electrical shock which can cause

fatal.

Immediate Action: Ensure the area is


cleared with no electrical cables. Provide
increased protection through the use of
an RCD. Provision to PPE which are non
conductors of electricity.

routine visual inspections avoid using in
wet conditions, carry out pre used checks
of the equipments for signs of damage.

turning on slopes, the edge of
excavation, falling of waste material due
to overload.

are suited for intended use and
environment and are maintained in safe
working order. Driver should be
competent. Banksmen should be used.

17. Excavation work that was carried


out was found unsafe because of
unsupported sides, which may lead to
the collapse of the structure, slip and
cave in, severe crush injuries due to
heavy soil, workers could get buried or
entrapped in soil can asphyxiate in
minutes.

Immediate Action: Sides can be


supported temporarily by a metal box that
can be lifted into the excavation and
moved along to give a protected work
area. Proper barricading around the area
along with proper supervision with PPE.

Long Term: The sides of the excavation


can be battered to avoid any soil to `slip;
the sides can be shored with metal or
timber thats fixed inside the excavation.

before start of work, after any accidental
fall of material, after any event likely to
affect strength and stability.

18. Witnessed public taking shortcuts


through the construction site due to
improper fencing which may lead to
injuries due to falling of objects, loss of
machinery or steal plant by thefts.
Distraction by the public to the workers

site access points, security staff should
be provided round the clock, secure all
portable equipments from theft.

Introduction including overview of area inspected and activities taking place

Fadi builders are located in jubail which covers an area of 100 acres. At present more than 500
workers are working with Fadi builders they include both organisational staff and also contractors. The
Revenue of Fadi Builders in India is about USD 5.4 Billion every year. The Health & safety practices
that Fadi Builders envision are good enough and they have a reputed image among the public.

Fadi Builders are currently undertaking a project that is situated in Jubail. The concept of this build is
to put up a luxury village along with a shopping mall, hospital and a recreation centre in the same
vicinity and give the people easy access to all amenities. The shopping mall construction site is the
very essence behind these observations, it was inspected on 10th December 2015 it has 3 floors and
is spread across 10 acres of land. It encompasses various machines that were scattered around the
site such as like Forklifts, numeric controlled crane, mobile elevated crane operated in and around the
construction building.

During the inspection, we looked into a number of things such as the how many persons
carry out a particular activity, and while doing so if they were wearing their protective equipment.
Identifying Hazard inside the workplace, Control measures are effective with proper HSMS like
Inspection checklist, Verifying proper maintenance are carrying for vehicles & equipments, Audit
Reports, Safety Committee report, Near-miss report, Incident report, Ill health data maintenance are
carrying as per the EHS Plan of the Organization, Interaction with contract employees' and site
engineers for ensuring all are aware of Policy and rules inside the site premises. The number of
Forklifts used inside the Site, Type of mobile operating elevated crane, numeric controlled crane,
electrical power supply, types of fire extinguisher, sanitation facilities, emergency procedure and

arrangements, machineries like portable driller, cutter machine involved in the productivity, first aid
and control measures.

Both the indoor and outdoor work activities that are carried out in the construction area has
10 emergency exits, separate yellow markings for pedestrians, signage, 30 fire extinguishers and 10
first aid kits. Its man power consisted of 30 welders, 10 technicians, 10 supervisors, 5 electricians
and layman, 5 banksmen, 4 engineers, 3 Contractors, 9 sub contractors, 20 brickmasons. There are
also 2 maintenance engineers and 1 site officer. In order to improve and set new health and safety
targets questionnaires are made on Health & Safety of an organisation.

Executive Summary
In this report, the clear analysis of Fadi Constructions, Jubail about risks involved in their work activity
such as working at height, Fire, Collision between vehicles and pedestrians and noise & vibration
produced by the machine, workers under the influence of alcohol and persuading the management to
control actions as suggested in this report. Overview of the following findings in the report.

1. Workers working at the height of 30 feet without any safety measures without any safety
precautions/measures are open to the risk of fall from height and consequences are
Fractures, breaking bones, or even death. Control Measures: usage of fall arrester belts,
nets and gaurd rails with adequate training and supervision.
2. Forklift driver found driving amoung the pedestrians as there was no separate pathways laid
out has the risk of collision with pedestrians and other vehicles and the consequences could
be a major injury, abrasion, breaking bones and/or even death including loss of organisational
property. Control measures: lay out separate pathways for vehicles and pedestrians,
competent driver, proper signage and provision to banksman.
3. Worker found fire fighting with no personal protective equipment and no access to fire
extinguishers and consequences could be burn, due to incorrect method to put out the fire it
may spread and harm the others around him. Control measures: provision to fire watcher
and fire marshal, adequate access to fire extinguishers, frequent fire evacuation drills and fire
detectors with periodical inspection around the work place.
4. Drillers and workers working on the concrete breaker were exposed to vibration and noise
respectively without any safety measures. They might tend to face the consequences of
vibration white finger, nervous damage, muscle damage due to vibration and noise induced
hearing loss, temporary ringing and tinnitus for due to noise. Control Measures:

Maintenance of the machine, building acoustic haven, provision to personal protective


equipments, job rotation and adequate training is advisable.
5. Workers found working under the influence of alcohol during working hours are shown to
faces certain consequences like Absenteeism, bad quality of work, reduction in work time,
and deterioration in working relationships. Control Measures: Drug and alcohol awareness
campaigns should be considered, arrangement for random alcohol test that workers will be
subject too. Disciplinary procedures for workers, who refuse. Explain the legal requirement
which states the reason why the worker should not be under the influence of alcohol.

All the control measures are mandatory to safeguard the health and safety of an organisation &
also reduce the cost incurred to an organisation by preventing the accidents happening. It also
satisfies the HEALTH AND SAFETY WORK ACT, 1974. From this we can improve positive health
and safety culture of an organisation and also environment

Main findings of the inspection


1. WORK AT HEIGHT:
The worker who was cleaning the building structure was using a ladder 30 feet in the air. He
was not provided with any personal protective equipments/devices and there was no sufficient
guard rails on the working platform. Therefore there is a likelihood of falling of the ladder, the
ladder toppling sideways, the ladder base slipping out from the wall, or objects slipping off
from the ladder. All of the mentioned hazards have the potential to cause severe injuries like
fractures, broken bones, brain damage and/even death.
The risks can be countered by using a mobile elevated working platform instead of a ladder to
work at 30feet height or fall arresting belts, place guard rails to the working platform, only
one person should climb the ladder, Ideally the ladder should be secured at the top, angle of
the ladder should ideally be 75 degree to the horizontal or at a ratio of 1:4 distance away from
the wall to height. Adequate inspection before start of work.

reduces the reputation, affect the business with clients and third parties, stake holders, and
employee morale has been affected. In order to avoid such consequences, organisation should
implement certain control measures like fall arrester belts and guard rails to protect them
from falling. These control measures can be implemented at the expense of 5,000 SR.

2. Collision:

The Forklift driver was found operating the vehicle among the pedestrians as no separate
pathway was provided as well as no banksman. The driver/operator has the potential to over
speed to cover his work time, reversing without the help of a banksman, blind spots such as
corners and entrance, obstructed visibility (overloaded forklift truck driving forwards), lack of
vehicle maintenance could cause break failure. This could result in a collusion with vehicles
and pedestrians that leads to major injuries to the pedestrians like fractures, abrasion even to
fatality and also collusion into organisational property which could cause loss/damage to
machineries.
The identified risk can be countered by providing vehicle free zones and pedestrian free zones,
vehicle traffic route layout can be implemented, Speed limit to vehicles, banksmen may be
used, crossing points may be implemented, appropriate signage, loud beacons, wear reflector
jackets, and the driver should be a competent person.

3. FIRE:

While putting out a fire the worker was not wearing any personal protective equipment and
had no access to fire extinguishers. The fire may have been caused because of hot work
which had been carried out, smoking in the premises, given the benefit of doubt to deliberate
ignition by haters, or due to some contact with heating appliances. This may cause skin burn
and irritation, panic, cardic arrest due to panic or even death of the worker if caught while fire
fighting with no training, if the fire spreads then it may have the potential to destroy the entire
construction site including other workers around the area.
The Identified risk can be countered by removing any flammable material from the work
area, items which cannot be removed are covered with fire-retardant blankets, proper

housekeeping, avoid any wooden floor which is damped down, provision to suitable fire
extinguisher at reach, provision for a fire-watcher in the area, fire detectors with panic alarms
and proper supervision of work area.

4. NOISE AND VIBRATION:


Noise and vibration (Cutting machine, drilling machine, grinding machine) Noise level above
110 decibel in the area. It causes temporary ringing, noise induced hearing loss (NIHL),
tinnitus and vibration causes vibration while finger, nerve damage joint damage to the
operator.

The identified risk can be controlled by the maintenance of machineries, lubricating machine
moving parts using oil, damping the machine parts by fixing rubber line coating at the bottom
and grounded properly, use acoustic haven, and job rotation , training to the machine
operator, investigate and inspect the machine installed.
Noise and vibration increase stress to the workers, insomnia, and health risks are temporary
ringing , noise induced hearing loss (NIHL), tinnitus and vibration cause vibration while
finger, nerve damage joint damage to the operator. Probably it will affect the productivity,
loss of staff from productive activity, affect the health condition of staff, and also increase the
criminal penalties.
So organisation should take the control measures mentioned above

such as job rotation, lubrication, maintenance, use of accosting haven and inspection to be
carried out periodically. The cost associated with these control measures are about 10,000/SR It will improve the health and safety of the organisation.

Workers were found under the influence of alcohol during work hours which may lead to
reduction in quality of work, reduction in work rate, dishonesty, theft, irritability, mood
swings, increase absenteeism and deterioration in working relationships.

policies and control measures like Drug and alcohol awareness campaigns should be

When the accident occurs it will directly or indirectly bring expenses to the company such as
Fines, compensation, criminal penalties, increased insurance premiums, and workers sick

such consequences, the company should implement such policy and measures to go run
awareness campaigns, these will not cost anything to the organisation. However, such good

Conclusions

I would like to take this opportunity to thank Fadi Builders for giving me the opportunity to work with
your organisation and for conducting a safety assessment on your construction site in Jubail. At the
start of my assessment, on entering the premises it was delightful to see such lively place to work, a
number of first aid kits, adequate wastage disposal operated, the restrooms were very hygienic,
whistle blower system was in place and a positive worker culture was cultivated. However there are
areas of concern which needs to be addresses at the earliest if they are not controlled then it may end
up in an accident which could cause the entire working of site to come to a halt, and the organization
may end up paying for the damages or losses, legal complication & business loss, etc.

Separate pathways to avoid collusion and banksmen for assistance.


The appropriate PPE required should be provided.
While working at heights there should be fall arresters and guardrails.
Proper training should be provided to all drivers/ vehicle operators.
Restricted speed limit for all vehicles.
Provide mock fire drills and adequate fire extinguishers around the site.
Maintenance of tools, vehicles and equipment should be periodically done.
Random testing for intoxication.
Sufficient lighting should be fixed.
Every day on completion of work through housekeeping should be done

All these activities have to be considered as priorities as they may lead to major accidents if the
control measures are not implemented.
The Organisation should ensure their duty of care to protect its employee, fulfil the legal
Requirements, satisfy the public (Societal) about health and safety practices in the organisation and
also to protect the environment. Economic considerations are an important factor because if it fails in
this department it would lose its good business reputation among contractors and the public, leading
to a loss of future tenders resulting in economic losses. Being a billion dollar organisation its market
value will be considerably reduced.
To give confidence to the workers and contractors that health and safety is a priority in the
organisation, I would recommend a general programme of inspection and some awareness initiative
done by the management; doing so will enumerate a positive vibe. It can be done easily by
supervision over hazardous work activities and give tool box talks about workplace health and safety
regularly. By imparting these safety measures the organisation would reach the health and safety
targets easily and is in tune with the health and safety culture as per recommendation by ILO-OSH
conventions and Occupational Health and Safety Act 2000.
