No more school logo/seal. Only the
official DepEd logo and seal will be used
Current practice:
Every school prints out a Form 137 with their
school logo and seal. If a student transfers to
another school, this form is stapled to a new
Form 137 from the new school.
Rationale:
One official form will be used.
If a student transfers schools, the new school will
continue filling up the original form.

The space for the school seal is for the schools to
know where to place the seal when they certify the
