Our Considerations
Form 137 for SHS should
Be simple and easy to
use (not a burden to
teachers)
Indicate the eligibility of
the learner for SHS
Indicate the track/strand
and subjects (core,
applied, specialized)
Show historical academic
record per semester and
the grades per quarter
(as in DO 8, s. 2015)

Be flexible enough in
case student transfers to
another track or another
school
Be able to monitor
student progress and
subjects taken (including
remediation)
Be professional looking if
it is to be used to apply
for jobs, colleges, or
further training

Features
Schools are provided with 2 options:
Print it out and fill it up manually
(current practice)
Use it as an electronic copy

Divided into 3 sections:

Features
No more school logo/seal. Only the
official DepEd logo and seal will be used
Current practice:
Every school prints out a Form 137 with their
school logo and seal. If a student transfers to
another school, this form is stapled to a new
Form 137 from the new school.
Rationale:
One official form will be used.
If a student transfers schools, the new school will
continue filling up the original form.

Parents names, occupation, home address


are removed from learners information
Rationale:
Parents names are not necessary (they are of age);
Parents occupation and home address are not good
identifiers because it can change

Rationale:
It should be noted that not only JHS completers or
graduates are eligible for SHS

transfers made by the student
using only one form.

Shows the Remedial Class Marks and the Recomputed Final Grade
Rationale:
Instead of a separate sheet being stapled to the form (which can get lost),
teachers can already write these on the form itself

Rationale:
Idea came from school samples.
Schools conduct a checking of
forms to make sure that all
information in the form are
correct. It adds validity to the
form.

Rationale:
To remove unnecessary additional items that teachers need to
write/encode. Report on attendance is already found in Form 138
(Report Card)

Track/Strand Accomplished and space for School


Seal can be found at the back

Rationale:
The Track/Strand Accomplished shows what the
student finished in SHS (esp if student shifted to
another track or strand during a sem)
The space for the school seal is for the schools to
know where to place the seal when they certify the
form

Note includes instructions for any erasures or
alterations and instructions for the school in case a
student transfers and when a student graduates
Rationale:
To address concerns that teachers have to rewrite the
form when they accidentally make mistakes
To guide the schools in what to do when a student
transfers or graduates from school
