Traumatic arterial occlusion following major or minor blunt trauma, especially in the absence
of any other bony injury, is a rare phenomenon [1]. Motor-scooter handlebar syndrome is
one such type of arterial occlusion affecting vascular structures following direct blow by a
handlebar of a motorbike or bicycle to the groin. Given their superficial course at this
location, femoral vessels are the most common vascular structures affected. In all instances,
injury to iliac vessels remains exceedingly rare, given its posterior position within the pelvis,
representing only 0.4% of vascular injuries [2].
In this case, we highlight a delayed presentation of external iliac artery occlusion secondary
to motor-scooter handlebar syndrome, in a paediatric patient. Pathophysiology and
management of vascular injuries in the paediatric population vary significantly compared
with the adult population. Additional factors which need to be considered include: smaller
vessel size or vessel spasm, higher risk of infection, tendency for re-stenosis and continuing
growth. We review previous paediatric cases of this unusual vascular injury to highlight the
pathology and most appropriate management option.
CASE REPORT
A 15-year-old male presented to the emergency department following a direct blow from his
bicycle handlebars to his groin. His chief complaint was pain and swelling in the right
inguinal region. His abdominal examination was unremarkable, except for a small abrasion
and visible mass in his right groin. Peripheral pulses were palpable bilaterally. After
exclusion of other abdominal or chest injuries, he was discharged on the same day with a
diagnosis of right groin haematoma and follow-up in 68 weeks.
He presented to his general practitioner 2 weeks later with claudication. His symptoms
included pain and paraesthesia in his right buttocks on mobilization >100 m. He was found to
have absent peripheral pulses in his right leg, although it appeared well perfused with a
normal capillary refill. An arterial Doppler ultrasound showed a right external iliac artery
thrombus occluding the proximal two-thirds of the vessel. A computed tomography scan of
the abdomen and pelvis with intravenous contrast identified complete occlusion of the right
external iliac artery 1 cm beyond its origin. However, the common femoral artery and
profunda femoris remained patent via collaterals (Fig. 1).

DISCUSSION
Most acute occlusions are evident within 24 h, and delayed presentations are uncommon. The
paediatric population have a greater capacity for the development of collaterals. As such,
patients may remain asymptomatic until a period of growth spurt or they resume more
rigorous physical activity. This can lead to delay in the diagnosis, and a high index of
suspicion needs to be maintained to avoid ischaemic complications such as limb length
discrepancy [8, 9]. The initial presentation may simply be that of extra-arterial haematoma. In
such instances, it is important to rule out any secondary occlusion of the femoral artery and
vein that may predispose to the development of thrombosis within these vessels. Duplex
sonography should be carried out to assess flow velocities and waveform characteristics in
these patients, and is especially suited to the paediatric population as they have reduced
abdominal fat, and there is no radiation. In addition, a study by Fry et al. [10] identified
ultrasound, in the right hands, to be highly sensitive (100%) and specific (97.5%) for the
detection of arterial injuries when compared with surgical findings.
Open arterial thromboendarterectomy with graft or patch repair is the standard of care for
such cases in trauma centres [10]. Previously documented cases include patch repair or
bypass using saphenous vein, bovine pericardium and synthetic material [13, 5, 6]. One case
was successfully treated with IV heparinzation alone [7]. However, in most injuries, the
presence of significant groin haematoma may limit anticoagulation, requiring more urgent
operative re-vascularization. In our patient, delayed presentation and presence of significant
collaterals meant definitive operative management could be planned as a semi-elective
procedure. The role of endovascular intervention in this paediatric population has previously
been documented mainly as a temporizing measure for revascularization [4, 5]. Angiletta et
al. [4] used a nitinol stent in the distal superficial femoral artery of a 13-year-old patient
whose arterial and venous calibre at the time of injury were of inappropriate size for
reconstruction. The main disadvantage of endovascular interventions in paediatric patients is
the constantly enlarging calibre of the vessel with a fixed stent diameter. This can predispose
to complications such as restenosis, stent fracture, stent dislocation and acute on chronic
ischaemia [4]. As a result, future vascular reconstruction may be compromised and in
extreme cases, this may progress to significant ischaemia and even limb loss. We provide an
unusual cause of external iliac occlusion secondary to bicycle handlebar injury to the groin.
All patients presenting with groin injury from this mechanism should be carefully
investigated with duplex sonography and monitored for risk of vascular injury. The presence
of collaterals, particularly in the paediatric population, can lead to delay in diagnosis.
Literature seems to advocate open primary surgery for management of these injuries;
however, endovascular and conservative medical management have also been used
successfully. The latter two were contraindicated in our patient and open surgery was the
most appropriate option. Long-term follow-up, beyond the 12-month period, is needed,
particularly in relation to known vascular complications including pseudoaneurysms,
arteriovenous fistula formation and restenosis.

Abstract
Oklusi arteri berikut trauma tumpul merupakan kejadian biasa. Kami melaporkan kasus yang
tidak biasa dari oklusi eksternal iliac artery tertunda dalam laki-laki muda setelah cedera
tumpul abdomen. Dia berhasil diobati dengan thromboendarterectomy dan saphena perbaikan
vena Patch. Hanya ada segelintir kasus yang terdokumentasi terjadi pada populasi pediatrik.
Semua pasien dengan cedera pangkal paha dari mekanisme ini harus hati-hati diselidiki dan
dimonitor untuk risiko cedera vaskular.
INTRODUCTION
Trauma arteri oklusi berikut trauma tumpul besar atau kecil, terutama dengan tidak adanya
cedera tulang lainnya, adalah fenomena langka [1]. 'Motor-scooter stang syndrome' adalah
salah satu jenis seperti oklusi arteri yang mempengaruhi struktur vaskular berikut pukulan
langsung oleh stang sepeda motor atau sepeda ke pangkal paha. Mengingat saja dangkal di
lokasi ini, pembuluh femoralis adalah struktur vaskular yang paling umum terpengaruh.
Dalam semua kasus, cedera pembuluh iliac tetap sangat langka, mengingat posisi posterior
dalam panggul, hanya mewakili 0,4% dari cedera vaskular [2].
Dalam hal ini, kami menyoroti presentasi tertunda dari iliac eksternal oklusi arteri sekunder
untuk 'motor-scooter stang sindrom', pada pasien anak. Patofisiologi dan manajemen cedera
vaskular pada populasi anak berbeda-beda secara signifikan dibandingkan dengan populasi
orang dewasa. faktor tambahan yang perlu diperhatikan antara lain: ukuran yang lebih kecil
kapal atau kapal kejang, risiko tinggi infeksi, kecenderungan untuk re-stenosis dan
pertumbuhan berkelanjutan. Kami meninjau kasus pediatrik sebelumnya cedera vaskular ini
tidak biasa untuk menyorot patologi dan opsi manajemen yang paling tepat.
CASE REPORT
Seorang laki-laki 15 tahun disajikan ke gawat darurat menyusul pukulan langsung dari setang
sepeda untuk selangkangannya. Keluhan utamanya adalah rasa sakit dan bengkak di daerah
inguinal kanan. Pemeriksaan perut nya biasa-biasa saja, kecuali untuk abrasi kecil dan massa
terlihat di selangkangan kanannya. denyut perifer yang teraba bilateral. Setelah pengecualian
dari cedera perut atau dada lainnya, ia diberhentikan pada hari yang sama dengan diagnosis
selangkangan hematoma yang tepat dan tindak lanjut dalam 6-8 minggu.
Dia disajikan kepada dokter umum nya 2 minggu kemudian dengan klaudikasio. gejalanya
termasuk nyeri dan parestesia di pantat kanannya mobilisasi> 100 m. Dia ditemukan
memiliki denyut perifer absen di kaki kanannya, meskipun muncul baik perfusi dengan
pengisian kapiler normal. USG Doppler arteri menunjukkan trombus arteri iliaka eksternal
yang tepat occluding proksimal dua pertiga dari kapal. Sebuah computed tomography scan
perut dan panggul dengan kontras intravena diidentifikasi oklusi lengkap dari arteri iliaka
eksternal yang tepat ~ 1 cm di luar asalnya. Namun, umum femoral arteri dan profunda
femoris tetap paten melalui agunan (Gambar. 1)

DISCUSSION
'Motor-scooter stang syndrome' adalah bentuk jarang dari arteri cedera tumpul setelah
pukulan langsung oleh stang sepeda motor atau sepeda [3]. Hanya segelintir kasus pediatrik
telah dilaporkan [1, 2, 4-7]. Kapal femoralis umum adalah struktur vaskular yang paling
sering terkena. Sebuah situs umum untuk cedera ini adalah pada ligamen inguinal, di mana
arteri femoral dangkal dan kursus anterior ke ramus pubis superior dan kepala femoral.
Dengan demikian, maka rentan terhadap kompresi antara stang dan struktur tulang posterior.
Selain itu, itu adalah struktur yang relatif bergerak, ditambatkan oleh cabang arteri, jaringan
ikat periadventitial dan selubung femoralis [8].
Beberapa mekanisme patologis telah diidentifikasi menyebabkan oklusi pembuluh tersebut.
Mungkin, yang paling umum adalah bahwa dijelaskan oleh Dajee et al. dalam mencatat kasus
pertama yang diketahui dari oklusi aorta akut menyusul cedera sabuk pengaman. Mereka
mengusulkan bahwa air mata keliling dari intima mengarah ke diseksi dan prolaps dari
lapisan aorta batin, menyebabkan oklusi lumen lengkap [8].
Kebanyakan oklusi akut yang jelas dalam waktu 24 jam, dan presentasi tertunda jarang
terjadi. Populasi anak memiliki kapasitas yang lebih besar untuk pengembangan jaminan.
Dengan demikian, pasien mungkin tetap asimtomatik sampai masa lonjakan pertumbuhan
atau mereka melanjutkan aktivitas fisik yang lebih ketat. Hal ini dapat menyebabkan
keterlambatan diagnosis, dan indeks kecurigaan yang tinggi perlu dipertahankan untuk
menghindari komplikasi iskemik seperti panjang tungkai perbedaan [8, 9]. Presentasi awal
mungkin hanya karena hematoma ekstra-arteri. Dalam hal demikian, penting untuk
menyingkirkan setiap oklusi sekunder dari arteri dan vena femoral yang mungkin predisposisi
perkembangan trombosis dalam kapal ini. Duplex sonografi harus dilakukan untuk menilai
kecepatan aliran dan karakteristik gelombang pada pasien ini, dan sangat cocok untuk
populasi anak karena mereka telah mengurangi lemak perut, dan tidak ada radiasi. Selain itu,
sebuah studi oleh Fry et al. [10] diidentifikasi USG, di tangan yang tepat, menjadi sangat
sensitif (100%) dan spesifik (97,5%) untuk mendeteksi cedera arteri bila dibandingkan
dengan temuan bedah.
Terbuka arteri thromboendarterectomy dengan graft atau perbaikan patch adalah standar
perawatan untuk kasus seperti di pusat-pusat trauma [10]. kasus sebelumnya
didokumentasikan meliputi perbaikan patch atau memotong menggunakan vena saphena,
pericardium sapi dan bahan sintetis [1-3, 5, 6]. Satu kasus berhasil diobati dengan
heparinzation IV saja [7]. Namun, di sebagian besar cedera, kehadiran selangkangan
hematoma signifikan dapat membatasi antikoagulasi, membutuhkan lebih mendesak operatif
re-vaskularisasi. Dalam pasien kami, tertunda presentasi dan kehadiran agunan signifikan
berarti manajemen operasi definitif dapat direncanakan sebagai prosedur semi-elektif. Peran
intervensi endovascular pada populasi pediatrik ini sebelumnya telah didokumentasikan
terutama sebagai ukuran raguan untuk revaskularisasi [4, 5]. Angiletta et al. [4] menggunakan
stent nitinol dalam arteri femoral dangkal distal dari pasien 13 tahun yang arteri dan vena
kaliber pada saat cedera adalah ukuran yang tidak pantas untuk rekonstruksi. Kerugian utama
dari intervensi endovascular pada pasien anak adalah kaliber terus memperbesar kapal
dengan diameter stent tetap. Hal ini dapat mempengaruhi komplikasi seperti restenosis,
fraktur stent, stent dislokasi dan akut pada iskemia kronis [4]. Akibatnya, rekonstruksi
vaskular yang akan datang dapat dikompromikan dan dalam kasus yang ekstrim, ini dapat
berkembang menjadi iskemia yang signifikan dan bahkan kehilangan anggota tubuh. Kami
menyediakan penyebab yang tidak biasa dari iliac oklusi eksternal sekunder cedera bicycle
handlebar ke pangkal paha. Semua pasien dengan cedera pangkal paha dari mekanisme ini
harus hati-hati diselidiki dengan sonografi dupleks dan dimonitor untuk risiko cedera
vaskular. Kehadiran agunan, khususnya pada populasi anak, dapat menyebabkan
keterlambatan diagnosis. Sastra tampaknya menganjurkan operasi primer terbuka untuk
pengelolaan cedera tersebut; Namun, endovascular dan manajemen medis konservatif juga
telah berhasil digunakan. Dua terakhir yang kontraindikasi pada pasien kami dan operasi
terbuka adalah pilihan yang paling tepat. jangka panjang tindak lanjut, di luar periode
12bulan, diperlukan, terutama dalam kaitannya dengan komplikasi vaskular dikenal termasuk
pseudoaneurysms, pembentukan fistula arteriovenosa dan restenosis.
