AFFIDAVIT

I, CHARMAINE G. ISIDRO, Filipino, of legal age, single

and a resident of Venus Street, Tisa Hills Village, Tisa,
Cebu City, after having been sworn to in accordance with
law hereby depose and state that:
1. That I am a Physical Therapy student of Velez College
with school address at F. Ramos Street, Cebu City, and
currently in my fourth year as a student of said
institution;
2. That I was enrolled in Research Methodology 1 (RM 1) in
the previous semester with classes during Tuesdays at
01:00PM- 04:00PM, under the advisory of Ms. Lariza
Fernando;
3. That I was marked a failing grade because of my
supposed absence on the 25th of November, 2014;
4. That I was in fact present on the said date;
5. That on the said date, I had some documents
photocopied before attending class and even came in
time for said class;
6. That I was present during the presentation of my
classmates (group mates) report;
7. That I am executing this affidavit to attest to the truth
of the foregoing facts and to request for a
reconsideration
of
my
final
grade
in
the
abovementioned course.
