TOPIC: The Constitution

Assessment:

Weaknesses
Struggles to stay on task. Hides in his
smartphone to avoid contact with other
students and teacher. Thinks Mr. Sav is a jerk
and social studies is boring (low motivation).
Student has been observed struggling in
social settings and gets nervous during lunch.
Has low handwriting skills.
Accommodations

Family Life:

Tested at a 7th grade reading level, scored a


325 on the 7th grade social studies state test
(300 is lowest score, 1200 is highest score).
Tested well on all other assessments.
Strengths
Loves videos, Youtube and Loves Minecraft.
Plays sports and enjoys active activities.
Drama class and art are his favorite classes.
Creates great works of art. Great at Math but
has low motivation. Dislikes Language Arts
but loves the teacher Mrs. Doe. Works. He
works best with one-on-one instruction.
Allowed to take breaks during class (5-minute
walk about outside the room), sits in the back
of class, can leave 3 minutes early for the
next class, and is allowed to work alone if he
chooses.
Stable home-life, middle class status.

needs to modify an assignment to meet the needs of this fictional student.
Create a storyboard that includes triggers, what happens when the trigger is registered,
questions the student will be able to answer and evidence of student completion (or
