the DAGYAW 2016 to be held in Sacred Heart Seminary on November 19-20, 2016.
I understand that the organizers exercise necessary safety precaution in the
duration of the youth event. In consideration of the benefits to be derived from the said
activity, I expressly waive any and all claims against the organizers on account of any
unforeseen accident or injury that my daughter/son might incur in connection with the
aforementioned youth gathering.
