JUDICIAL AFFIDAVIT

I, Arya S. Santiago , 25 years old, Filipino, single and a resident of #15 Saint Joseph St.
San Carlos Heights , Baguio City, after having been duly sworn to in accordance with the law,
depose and state:
In relation to the above-entitled case, the counsel Atty. Rolly D. Herida Jr at his office
located at Room 309 Laperal Building, Session Road, Baguio City, asked me questions which I
answered while I am at her office, fully conscious that I did so under oath that I may face perjury
and any other criminal liability for false testimony.
The questions asked by Atty. Micah Felizardo and the answers I gave are as follows, to
wit:
