OBJECTIVE
To set out the procedure to be followed before any person, as defined in the
Company Safety Rules, is appointed to carry out specified tasks under the
Rules.
2.
SCOPE
This Management Instruction lays down requirements for the appointment
of persons concerned with ensuring Safety from the System under the
Company Safety Rules. The categories of Authorisation are as follows:

DEFINITIONS
Definitions printed in bold type are defined in the Company Safety Rules.

GENERAL
The appointment of persons is published in the form of a Register of Defined
Persons, (Appendix 1) held by the Site Managers. Copies of the Register are
maintained in the designated Permit Office(s).

Responsibility
Each Manager is responsible for compiling lists of persons under his control
who he wishes to appoint as a Safety Controller, Issuer, Safety Coordinator or Designated Person. In making this first stage of nomination,
the Manager must be satisfied that each of the persons selected is suitably
qualified and experienced for the responsibilities envisaged.
APPOINTMENT OF SAFETY CONTROLLERS, ISSUERS, SAFETY
CO-ORDINATORS AND DESIGNATED PERSONS
General
The appointment of any Person as a Safety Controller, Issuer, Safety
Co-ordinator or Designated Person is subject to the candidate being
examined formally by an Authorised Panel.
Duties which require that a person be a Safety Controller, Issuer,
Safety Co-ordinator or Designated Person are specified in the
Company Safety Rules, Codes of Practice, Management Instructions and
Local Safety Instructions.

Authorisation Form
Upon satisfactory completion of the authorisation interview, the panel
Chairman shall ensure that the Authorisation Form is fully completed and
that it specifies the agreed limits of authorisation. (Appendix 2)

Rules to discharge his/her duties as a Safety Controller in a safe and
satisfactory manner.

The Authorisation Panel must determine that each candidate has the
required knowledge of the local plant, safety procedures and the Safety
Rules to discharge his/her duties as an Issuer in a safe and satisfactory
manner.
Safety Co-ordinators
Safety Co-ordinators will normally be nominated by the Site Manager (or
his nominee), additional training on the duties and responsibilities of a
Safety Co-ordinator shall be given where necessary.
Designated Persons
Panel Assessment
The Authorisation Panel must determine that each candidate has the
required knowledge of the local plant, safety procedures and the Safety
Rules to discharge his/her duties as a Designated Person in a safe and
satisfactory manner.
WITHDRAWAL OF AUTHORISATION
When a Persons Authorisation is withdrawn for any reason, the Site
Manager should notify all persons holding copies of the certificate. If the
Person is to be subsequently re-authorised, he/she should be re-trained,
tested and re-examined by an Authorisation Panel, in accordance with
paragraphs 6.1 - 6.5.

CATEGORIES OF AUTHORISATION
Safety Controller
Responsible for ensuring that adequate precautions are take to provide
Safety from the System whilst work is carried out. Capability to prepare,
issue, transfer, suspend and cancel Safety Documents.
Safety Co-ordinator
To Co-ordinate safety precautions at Company interconnections with other
Companies Plant and Apparatus or with other safety co-ordinators.
Capability to Prepare, Issue, Transfer, suspend and Cancel Safety
Documents.
Issuers
Responsible for the issue and transfer of specified Safety Documents.
