Cell block

a) Cell block is paraffin block that embedded with sediments of fluids.

b) Any cytologic samples can be processed into cell block but are done after completion of
cytologic sample.
c) The advantages of cell blocks are provide additional diagnose for cytologic sample
which means having better diagnosis.
d) Another advantage is gain multiple sections of the same material may be processed for
routine stain (Hematoxylin & Eosin ) and special stain.
Screening and reporting
a) The repoting of cytologic samples are according to the Bethesda System (TBS) which is
the system for reporting cervical and vaginal cytologic diagnoses for reporting Pap
smear results.
b) Screening vertically is best suggested because the slide overlapping and no cell will be
missed compared to screen horizontally which might produce visual fatigue.
c) Marking process is done to facilitate the examination of cells on multiple occasion or by
several observant.
d) A cytologic report should have these following description :
Specimen type : whether conventional smear (Pap smear) or liquid-based
preparation or others.
Specimen adequacy : Satisfactory for evaluation or unsatisfactory for evaluation
or specimen rejected or not process. Give specific reasons.
General categorization :
Negative for Intraepithelial Lesion or Malignancy (NILM) which is no cellular
evidence of neoplasia and have organisms such as fungal or bacteria and other
Non Neoplastic findings such as atrophy or glandular cells status post
hysterectomy.
Epithelial Cell Abnormality which is squamous cells such as atypical
squamous cells , LGSIL, HGSIL and glandular cells such as atypical,
adenocarcinoma and endocervical adenocarcinoma in situ.
Other malignant neoplasm :
Ancillary Testing which provide a brief description of the test method and report
the result so that it is easily understood by the clinician.
Automated review, specify the device and result.
Educational notes and suggestion. Suggestions should be concise and
consistent

Title : Screening and reporting
Objectives : Able to understand and explain about cytologic screening and reporting according
to Bethesda System.
Materials and apparatus :
a) Microscope
b) Fine point pen
c) Slide

Procedure :
a.

e.
f.

Place the slide on mechanical stage.

The label on same side either left or right.
The slide are screened vertically or horizontal.
Screening done at 10x objective and 10x ocular lens of microscope.
Details of the examination of cells are done at 40x objective.
Marking are done when seen abnormal cells by using fine point pen by place an ink dot

net to the cells.

g. A circle may be placed around the cells in the same way of using fine point pen. The
position of mark must be in relation to the cells.
h. Screening must done by two Medical Lab Technologists and validated by
Cytopathologists.
Results :

Glass slide

Light microscope

CONCLUSION

In the end of this practical report, I am able to understand the overall process that are
done in Cytology Laboratory. I am able to identify every instruments used in specific process
and their principle with mechanism processing. These instruments have facilitate in sample
collection and processing thus serve for better diagnosis in Cytology laboratory. I am also gain

knowledge of the advantages and disadvantages in every method used in lab and able to
compared it. Last but not least, I am able to perform the processing of Cytology sample in
practical lab and have more understanding from specimen reception until reporting the results.
