CASE FOR THE ROLE PLAY

Mental Health Case
Jane is a 28-year-old single female. She states that her boyfriend of two months asked her to
come to counseling when he found out about her cutting behavior. She scratches her arms with a
plastic knife whenever she becomes angry or feels out of control. This has occurred for at least
one year. She denies any suicidal thoughts and states that she does not want to die but does want
to be released from the emotional pain and anxiety.
Jane reports a history of persistent sexual abuse by her minister from the ages of 14 to 16. The
abuse stopped when he was transferred to another parish. She tells the story of the events without
any emotion. She has experienced nightmares of the event periodically since the event occurred.
She has difficulty falling asleep (D.F.A.) because she fears the nightmares. She avoids going to
church and states that the cutting behavior helps her to avoid thoughts of the abuse.

Two month ago, Jane scratched arm with plastic knife. Jane Boyfriend found her when
she scratch her arm herself. Janes boyfriend stopped her act, and he asked Jane for consult to
psychiatrist. But Jane rejected.
After that incident, Jane know about her condition and start to open her mind finally. She
want to go to hospital with her Boyfriend.
In the hospital (in the morning around 08.00 AM). Jane and her boyfriend go to the counceling
room.
Tok-tok-tok (Janes boyfriend knock of the door)
Doctor

: please come in. sit down please

Candra

: okey doc, thankyou

Doctor

: can I help you Mr.. what is your name?

Candra

: My name is Candra, and she is my girlfriend, Jane. We want to consult

with you about her problem.

Doctor

: oke. before that, I want to introduce myself. My name is Doctor Daeng

Muhammad Rafiqi, you can call me Doctor Daeng. I want to give you
counseling. and your information will be secret.

Jane

: okay doc.

Doctor

: oke what is your problem Jane? Can you tell me

Jane

: doc.. actually two months ago my Boyfriend told me to consult. But I

didnt want to go. Because Im in okay.

Dokter

: So . How can you came to me finally?

Jane

: em.. I want to know more about my mental health condition.

Dokter

: Okay, thats a good decision, we will try to help you and we will discuss
about your problem. Can you explain about your feeling?

Jane

: Two months ago, I scratched my arms with a plastic knife whenever I

becomes angry or feels out of control to forget it.

Dokter

: what is the event ?

Jane

: At 14 years old, I had the sexual abuse and I often do the strach, so I can
distraction my emotion.

Dokter

: Well . I understand what do you feel. Now we will observe your

condition. So that you can get the next treatment.

Jane

: okay doc.

After Jane moved to Pscyciatric Intensive Care Unit, Jane scratched her arm. And Candra
saw what do Jane. Candra reported it to the nurse ...
Candra

: nurse, jane doing scratching her arm.

Nurse A

: oh yes Mr., I will give her chlorpromazine injection. Can you help us to
give the medicine ?

Jane Boyfriend

: yes nurse. Okay.

Nurse A gives injection drug to Jane, assisted by Candra and nurse B. Jane calmed down.
After the implementation, the nurse and Candra go to the nursing station.
Nurse A

: Mr., Jane get sexual abuse and thats her traumatic event, I guess. When
it happen?

Candra

: since she was 14 years old until 16 years old.

Nurse A

: when her scratch behavior appears?

Candra

: actually the habit since 1 years ago , but I see her starch her arm since
two months ago . Today, she want go to the hospital to consult.

Nurse A

: ooh ... can you tell me clearly Sir ?

Candra

: Jane says that sexual abuse by the minister. And incident stopped when
the minister moved to another parish . She also has trouble sleep because
her fear of a nightmare and she always avoided go to parish.

Nurse A

: as prescribed, I will give you information about the drugs on the recipe.
First is chlorpromazine the function to sedative the condition.

Candra

: okay nurse

Nurse A

: next the intervention we will observation the Jane condition.

Candra

: okay, I need your help nurse, Im very confuse because of its problem.

Nurse A

: okay Mr.

After several four hours later, Nurse B observe the Janes condition (jane seems better)
Nurse B

: good afternoon Mrs. Jane

Jane

: afternoon.

Nurse B

: first I will introduce myself. My name is dian luberiono, you can call me
nurse dian. How are you?

Jane

: its much better than before nurse.

Nurse B

: here, I will talk with you about you. Are you agree?

Jane

: okay

Nurse B

: I need 10 until 15 minute. Are you agree?

Jane

: yes. Im agree .

Nurse B

: do you remember, where are you now?

Jane

: (silent)

Nurse B

: why you were here?

Jane

: I usually scratch my arms with plastic knife. When I remember the event.

Nurse B

: why?

Jane

: 14 years ago, I had experience about sexual abuse and the minister do it.
I scratced my arm. so I can forget the incident.

Nurse B

: you can distraction your feeling

Jane

: what is it, nurse?

Nurse B

: well, you can do your hobby or another activity when the traumatic
appear.

Jane

: my hobby is embroider . Can I do it to decrease my negative feeling?

Nurse B

: wow. Thats a good hobby. You can do that here or at your home.

Jane

: okay, I will try. But I dont have the tools and material, nurse.

Nurse B

: be relax. We have the tools and material for embroider.

Jane

: okay

Nurse B

: well, because the time is over, I think it is enough.. Before I go to the

nurse station, do you have any question?

Jane

: no nurse thank you.

Nurse B

: okay. Thanks for your cooperation. See you next time at 08.00 AM.
Tomorrow I will visit with doctor to look your condition . Are you agree ?

Jane

: Okay Nurse.

The next day, doctor visit in Psychiatric Intensiv Care Unit

Doctor

: good morning, nurse.

Nurse A

: morning doc

In client room
Nurse A

: this is the last doc. She is Jane. Before, she have self harm.

: the client said that she feel the condition better.

: general condition is good, client cooperative , vital sign( blood pressure:

120/70mmHg, pulse 60x/minute, respiration : 19x/minute, temperature
36C. medicine : chlorpromazine.

: the self harm are resolved.

: observation the condition, give medicine chlorpromazine, diazepam,

fluoxentine.

Doctor

: Nurse, the client can go home, before that you can give Jane and Janes family
about treatment to prevent self harm.

Nurse A

: okay doc.

After Candra completing administrative, Jane and Candra go to the nurses station to get
health education.
Nurse B

: well, Mrs Jane you can go home today.

Jane

: okay nurse.

Nurse B

: before you go home I will give you and your boyfriend information for
prevent cutting behavior.

Jane Boyfriend

: okay nurse

Nurse B

: previously, Mss Jane teach about implementation strategy for prevent

cutting behavior.

Jane

: yes nurse

Nurse B

: Mr. you must keeping safe environment of jane. The other strategy is
,take a deep breath to make you be relax, second you can do activity that
you like, third you can invite the other person to assertive communication,
fourth with some a drug and less with spiritual.

Jane Boyfriend

: okay nurse. What the kind and function the drugs?

Nurse B

: First, this is chlorpromazine the function make you relax and sleep early.
The color is orange. You can consume if necessary.

Jane Boyfriend

: oh okay nurse.

Nurse B

: second, this is diazepam the function for decrease anxiety. The color is
white. And third is paroxetine the function is anti depressant, the color is
pink.

Jane

: okay nurse.

Boyfriend

Jane

: How step to consumption the drugs?

Nurse B

: the drugs can consume after Mrs. Jane eat. And consume every 3 times a
day.

Jane

: ooh I see. Thank you nurse.

Nurse B

: okay , your welcome. So, you can check your condition at October 2 th
2016.

Jane

: okay, thank you.
