I, JOSELITO D. SANTOS, of legal age, Filipino, and with postal

Zealand, after being sworn to in accordance with law, do hereby
depose and say:
1. I was born on 01 July 1975 in Pasig City, Philippines;
2. I am 41 years of age and have resided in New Zealand since
__________;
3. I and Jane Buenaventura Arabit started dating sometime on
__________;
4. We officially became boyfriend and girlfriends on 29 June 2007;
5. After two years of being in a relationship or on 25 September 2009,
Jane and I got married at Saint John the Baptist Parish Church in Taytay,
Rizal, Philippines;
6. After we got married, my wife adopted my surname and has been
using the name Jane Arabit Santos in her official documents since
then;
7. We share the same regular and permanent residence in the
Philippines at 179 Lakeshore drive Street, Barangay Kay-buto, Tanay,
Rizal;
8. We are jointly responsible for basic living expenses of our two
dependent children;
9. Our first child, Jandi Arabit Santos, was born on 16 January 2011
in Taytay, Rizal;
10. Our second child, Bj Arabit Santos, was born on 16 October 2014
in Morong, Rizal;
11. My wife and I are not married to anyone else;
12. That I am executing this affidavit to attest to the truthfulness of the
foregoing facts.
