Relevant Parties hereto have appended the signatures of their representatives to the minutes of
previous weekly meeting.
(2) Discussion and Agreement
The Engineer requested the Contractor to report on progress of the previous week and schedule
for the next week with focal points on outstanding issues and solutions. Please refer to the
Contractors Construction Volume Report for the first week of November 2016 for more details.
Upon the Contractors report on actual construction progress, the following contents have been
discussed and agreed among all relevant Parties:
(i) Intake
Open and underground excavation for Intake Gate Shaft
The Contractor committed to complete all remaining volume (about 6000 m 3 downwards to
EL.1014) by November 30, 2016.
Construction safety at Intake Gate Shaft
About 200m hanging cable has been ordered by the Contractor to replace degraded ones for
dump trucks and excavator.
The Contractor committed to promptly install safety fence at EL.1042 to secure the construction
safety in this area.

Ventilation system has been installed but not yet operated. A steel pipe is suggested to direct the
air flow from the ventilation system downwards tunnel areas. According to the Contractors
confirmation, a backup ventilator is available in their warehouse.
Before sending their staff and workers down the shaft, the Contractor shall estimate the correct
time (at least 10-15 mins) and amount of air flow/oxygen level required for normal working
conditions. Next, a qualified oxygen meter shall be used to check whether the current oxygen
ratio is sufficient enough. Contractors workers shall go down and enter the tunnel one by one to
avoid mass fainting.
Slope protection for Intake:
According to the Employer, precast concrete slabs fabricated and installed for intakes slope
protection are unqualified in terms of compacting quality. In addition, the Contractor is advised
not to install tie bars before removing all soil in the adjoined corners of concrete slabs.
Where the slope is covered by open rock, the Contractor is required to leave the rock surface
intact without any shotcreting.
Change of method statement for (1) intake to surge tanks vertical shaft and (2) surge tanks
vertical shaft to dismantling chamber.
According to the bidding documents, excavation shall be started separately from the vertical
shaft to the upstream and downstream. The Contractor is planning to propose an alternative that
allows the excavation to be initiated from the upstream to the downstream. The method
statement of excavation and working drawings for these two sections shall be submitted to the
Engineer by November 14, 2016 and November 15, 2016, respectively.
Upstream Cofferdam:
The Contractor is advised to speed up the construction of temporary upstream cofferdam and
finish earlier next year so that the temporary cofferdam can be removed prior to October 2017
the month of highest flood water level, or else they would have to wait till next year (probably
till June 2018 when the construction period of package 12.01 is already over).
Magazine Warehouse:
No facilities are allowed to exist in a radius of 430 m from the Contractors Magazine
Warehouse. However, a police station, the Contractors camping site and a contractor of another
project (dredging and recovery of sand, grit and gravel) trespassed this safety radius. The
Employer has taken immediate actions such as direct contact with police station, relocation of
camping site and installation of mooring buoy lines for restriction.
(ii) Headrace Tunnel
Headrace tunnel and dismantling chamber
It is only 20 days left to the start of headrace tunnel and dismantling chamber excavation;
working drawings and planning for the excavation, rock bolting and shotcrete of such items
must be duly submitted for the Employer and Engineers review and approval.
Identification of tunnels endpoint and marking of blasting for next blasts shall be proper
executed by the Contractor.
Any oblique drilling holes shall be re-drilled in perpendicular direction in accordance with the
Employer and Engineers instructions, using a longer rod boring machine that will be delivered
to the site next week.
During the waiting time for the next blasting, the Contractor shall focus on primary and

secondary shotcreting. Steel mesh installation is required after the primary shotcreting, then the
Contractor shall proceed to the secondary shotcreting. Any non-compliance, including but not
limited to insufficient shotcreting, shall result in temporary suspension and repetition of primary
shotcreting.
On Sunday morning (November 6, 2016), the Contractor charged the drilling holes with
dynamites without prior written consent of the Engineer. Although the Contractor has stopped
the blasting under the Engineers instructions, the Engineer still expressed special concerns
about the Contractors dynamite management and dispatching procedures. The Contractor
admitted such non-compliance and committed to strictly manage and control explosives,
dynamites and alike. In addition, the charged bored holes shall be guarded to avoid potential
loss or accidents.
Lighting system is not properly supplied and installed. Electrical wires coating consists of only
one layer, which may result in electrical fire risk. Therefore, electrical wire must be properly
installed in a conduit. In addition, drawings of electrical system, brackets for ventilators, etc. are
also to be duly prepared and submitted by the Contractor.
Inexperienced tunnel engineers and workers, frequent downtime of boring machine, insufficient
shotcrete amount, improper supporting for loosen rocks, no fore-bolting, insufficient depth of
boring holes (only 3.3 m) etc. are outstanding issues to be duly remedied by the Contractor.
Otherwise, the Employer shall suspend the construction works at the headrace tunnel recurring
non-compliance activities are observed.
Slope protection from EL.1040: The Contractor shall check and evaluate the current conditions,
especially for T1 slope (rusty steel).
(iii) Valve House
The Contractor shall evaluate the current conditions of the slopes from EL. 999 and prepare the
method statement for rehabilitation of the existing access road to valve house as specified in Item
A9 of BOQ.
(iv) Penstock
Slope protection: The Contractor shall check and evaluate the current conditions of all slopes at
the penstock.
Blasting method statement: The Employer acknowledged to receive the blasting method statement.
However, the Contractor shall strictly adopt manual excavation method during the review and
consideration of the Employer.
Method statement for BL1-BL7: The Employer requested the Contractor to submit the method
statement for BL1-BL7 and the Engineer to review and approve it in a timely manner for early
start of excavation in December, 2016.
Safety fence: The Contractor is performing the installation of safety fence along the penstock,
starting from November 08, 2016.
A meeting on construction safety for BL1-BL7 shall be arranged later.
(3) Any other business
To leverage the advantages of incoming dry months, the Contractor is required to mobilize
additional manpower, equipment and machinery to accelerate the construction progress in
compensation to the current delay, speed up the submission of construction method statements
for headrace tunnel, surge tanks vertical shaft, intake gate shaft, dismantling chamber, etc. for
the Engineers timely review and approval.

A joint inspection between the Employer and the Engineer shall be conducted in early
December, 2016 to evaluate the current conditions of all existing slopes.
EVN has official instructions to the Employer and the Contractor to prepare the working
drawings of Unit 6. However, the drawings of underground equipment and technologies
prepared by Package 12.03 Contractor must be approved before the Employer can share such
drawings to the Package 12.01 Contractor to facilitate the design works.
The Employer instructed the Contractor to prepare necessary documents for acceptance and
payment of Interim Payment No. 3 by November 15, 2016. However, due to insufficient volume
executed, the Contractor committed to finalize all documents and submit to the Employer by the
end of November 2016.
