SAFETY RISK ASSESSMENT AND MANAGEMENT PLAN

Fill in more rows if required

1. Note each hazard that may potentially exist for the activity/task.
2. Determine and record a risk rating with reference to the Risk Calculator.
3. In the comments box, describe the hazard, and when and where the hazard is present,
and who will be affected.
4. Specify the risk control type from the Hierarchy of Control (top right).
5. Provide a control description for each current or proposed risk control.
6 Reassess risk with controls in place.

Electric Shock

15

During our 2-hour workshop

session held once a week. This
corresponds to sessions that
involve handling of circuit boards
in the process of making the
rocket launch pad.

RESIDUAL RISK SCORE

= R

PPE

Wearing rubber gloves to

prevent the risk of electric
shock.

0.5

Superficial injury due to

carelessness. Use of scissors is
limited to one or two workshops
involving the cutting of the
plastic bottle for the frame of the
rocket.

EL

The removal of distraction

ie. Working in solitude and
isolating the person at task
of cutting.

During field-testing, injury may

arise if not careful as the rocket
is launched. The high pressure
associated with the launch could
be dangerous. Field-testing is
limited to one workshop session
thus exposure is minimal.

AD

Notify and warn all

surrounding audience to
leave a reasonable
distance between
themselves and the launch
site.

10

Rocket exploding
during launch

15

30

Due to the high amount of air

pressure we are using to launch
the rocket, there are chances of
the plastic coke bottle exploding
due to buildup of pressure.

EN

Making sure to have

enough opening available,
to reduce chances of
buildup in pressure.

15

0.5

15

Circuit malfunction
during launch

15

45

Injury caused by carelessness in

constructing the circuit or faulty
electrical components that were
not checked causing rocket to
backfire or the launch to
malfunction.

AD

Ensure that all pieces of the

circuit are in good quality
working condition, and
have the circuit checked by
other people to ensure that
it is assembled correctly.

15

15

Damage to rocket
before launch causing
it to fly into a crowd

15

75

There is a risk associated to the

storage of the plastic bottle. If it
is punctured before flight, then
an unpredicted flight may result,
as there is a secondary exit for
air. It could fly into a crowd of
people, potentially causing nonfatal injury. Can potentially
cause significant damage
dependent on the design of the
rocket (sharp points).

AD

Store the rockets in an

environment where
damage to them is minimal.
Ensure the material of the
bottle is resistant to
damage. Remove potential
risks to the bottles from
their environment (ie
scissors).
Notify spectators, and leave
distance between them and
the launch.

15

0.5

37.5

Injury from the

materials

15

Its possible that somebody could

either cut themselves on sharp
material like wires or cause
bruising from heavy objects. The
platform will be lifted at some
stage and well be working with
wires and other sharp materials
at stages across the project. If
would take some carelessness
but its possible to happen.

PPE

Wear gloves to protect your

hands from scratches and
cuts as well as full-length
sleeves to protect your
forearms. Wearing stealcapped boots will mean
dropping heavy objects will
cause little pain.

Sunburn

10

60

During field-testing, sunburn is

highly likely to occur to any
person standing directly under
the sun. Although field-testing
occurs only for a short period of
time, exposure to sunburn is
quite likely given the strength of
the sun in Perth.

PPE

Wearing hats or applying

sunscreen should help to
reduce the risk of sunburn.

10

10

Having small material

flying toward
someones eyes

15

When composing the circuit

board, side cutters are available
for the cutting of wires to allow
easy fitting of components.
During the cutting process, if not
careful, off-cut could fly upwards
into someones eyes.

PPE
AD

Wearing safety glasses and

suggesting for the person
cutting the wire to angle the
wire downwards could
minimize the risk of injury.
