BIOGRAPHICAL SKETCH
The trainee was born at Bailan District Hospital, Bailan,Pontevdra,Capiz on July 22,
1995. She is the youngest child among the five children of Mr. RudrigoAdrias and Ms. Dahlia
Gubio. She isnow currently living in her Titas house at Campo Street, President Roxas, Capiz.
Rose Ann finished her Elementary Education at President Roxas West Elementary School in the
year 2007 and her Secondary Education at Cabugcabug National High School. She took up her
college degree Bachelor of Science in Business Administration major in Human Resource
Development Management at Capiz State University, Pontevedra Campus. She underwent her

Internship or On-The-Job training is one way by which students are given an opportunity
toapply the theories and principles that have learned from school. It also helps the student to
obtainapplicable knowledge and skills by performing in actual work setting. Colleges and
universities require their students to undergo such training within a specific number of hours as
part of the curriculum.
The trainee conducted her On-the-Job Training atRoxas Memorial Provincial
Hospital(RMPH) located at Arnaldo Boulevard, Roxas City, from November 9, 2015 until
February 17, 2016.
During the training, the trainee was exposed to variety of experiences and several tasks
like preparing anddistributing papers and documents for meeting, maintaining filing system,
answering phone calls and queries, photocopying, printing and encoding some files like Cash
Collection, PCSO,and DOH Fund.
The trainee applied her knowledge learnt from school. Some of these are
ComputerSubjects, English, and Management.
The trainee conclude that numerous number of patients admitted there every day but
facilities is not enough for the daily use and also the fund is not enough for the daily expenses of
hospital.
And she recommend the area need to purchase such facilities and equipment to improve
their system, conduct a seminar for all employees upgrade the system in order to cope with the
new government regulations,

dissatisfaction of the clients.
After the training the trainee learned a lot of things such as being an independent and
responsible to do the tasks assigned to her.

CHAPTER I
INTRODUCTION
Capiz State University is an institution of higher learning which offers several courses. It
aims to providequality education to the students. It has following functions: instruction, research,
production and extension.One of the courses offered at Capiz State University is the Bachelor of
Science in BusinessAdministration major in Human Resource Development Management.It is a
four year course which aims to equip students with the skills and knowledge to become
productive entrepreneurs, good-planners and business oriented individual.
On-The-Job training was included as requirements for graduation. It is one way of
exposing students tothe realities of work in operational institutions wherein the theories learned
from the school arebrought into practice.
On-the-Job Training is necessary to enhance the knowledge and skills of the students not
only the theoriesbut also in reality. It also aims to develop the desired skills among students as
well as apply classroom learning in actual practice.
CHAPTER II
Name and Place of Work
The trainee was assigned at Chief Administrative Office, Roxas Memorial Provincial
Hospital located at ArnaldoBoulevard, Roxas City, Capiz.
History of Company

The Roxas Memorial Provincial Hospital is the only Level II government hospital in the
province of Capiz and it operates under the regulation of the Department of Health. It is
accredited by the Philippine Health Insurance Corporation, providing safe, accessible and quality
public health care services.
RMPH caters to the health needs of the population of Capiz and the nearby towns of
Northern Iloilo, Carles, Estancia, Balasan, Sara and the Southern parts of Aklan, Batan and
Altavas.
The hospital has an authorized bed capacity of 100. However, due to the huge population
it caters and the surge of patients, it increases its bed capacity to accommodate a daily average of
173 admitted patients in the year 2014. In the same year, the rate of admitted patients has
increased having a total of 12,518 patients as compared to the 10,072 patients of the previous
year.
In November of 2013, Outpatient services were transferred to the new site at Capitol
Hills, Lanot, Roxas City. The decrease in the number of OPD consultations for 2014 may be
attributed to this move, since the new location of the OPD is a considerable distance away from
the main hospital and the center of the city.As a Level II hospital, it employs medical specialists,
qualified ancillary and nursing service personnel capable of providing the following services:

CHAPTER III
DESCRIPTION OF WORK
The trainee was assigned at Roxas Memorial Provincial Hospital. During the training, the
trainee was exposed to variety of experiences and several tasks at Chief Administration Office
which ranges from simple to difficult yet challenging. The entire 500 hours of On-The-Job
training was successfully devoted to the following tasks:
Maintaining Filing System

The first assignment for the trainee was to maintain the filing section in the office.
Recording all outgoing documents
Another task given to the trainee was recording all outgoing papers of RMPH before
transmitting it to Capitol.
Transmitting the report
The trainee was also assigned to transmit or deliver the report from Maam Lina to other
employee of RMPH.
Accommodate Customers
The trainee also accommodates the folks if Maam Lina was not around.
Sorting
The trainee was tasked to sort all financial statements and financial assistance report so
that it will be easy to find if needed.
Answering Phone and Queries
The trainee was assigned to answer the phone calls if there is no one who can answer it.
Photocopying
The Trainee photocopies the needed documents for the office copy.
Encoding and Printing

The trainee enhance her typing skill and familiarized the proper use of keyboard and
printer.
Other tasks
Other tasks assigned to the trainee were arranging the promissory notes, staple the
financial assistance and circulate memoranda.
DEFINITION OF TERMS
Administration is a group of people who manage the way a company, school, or other
organization functions.
Biometrics is a device used to store particular personal information about a person. It is
use for logging in and out of the institution as a basis for attendance on a daily time record.
Birth Certificate an official document issued to record a person's birth, including such
identifying data as name, gender, date of birth, place of birth, and parentage.
Calculator is something used for making mathematical calculations, in particular a small
electronic device with a keyboard and a visual display.
Computeris an electronic device for storing and processing data, typically in binary
form, according to instructions given to it in a variable program.
Email messages distributed by electronic means from one computer user to one or more
recipients via a network.
Hospital is a place where sick and injured people are given care of treatment and where
children are often born.

Promissory Note is a signed document containing a written promise to pay a stated sum
to a specified person or the bearer at a specified date or on demand.
Reimbursement is an act of compensating someone for an expense. Often, a person is
reimbursed for out-of-pocket expenses when the person incurs those expenses through
employment or in an account of carrying out the duties for another party or member.
Voucher a small printed piece of paper that entitles the holder to a discount or that may
be exchanged for goods or services.

MAJOR LEARNING DERIVED FROM OJT


Learning is not only inside the classroom but also outside.One of the best examples is the
On-The-Job Training which helps student to acquire more skills, experiences and knowledge. The
major learnings acquired by the trainee during her training were as follows: arranging the filing
system in the office, proper answering of telephone and quires, how to record outgoing papers,
how to make a flowchart, printing daily reports, how to label the DTR, rate the IPCR, segregate
the financial statement and financial assistance, and how to write a letter. Label the monthly
schedule of employee, review the Payroll of employee, how to use printer and computer in a
proper way, she also learned to follow instructions as what the supervisors said and to accept
opinion that they suggested.
CLASS MATERIAL USED IN THE TRAINING
Classroom learning was proven helpful in the conduct of the training. Theories and
principles in thissubject were practiced:

Computer Education
The Trainee is sitting at the computer table of her supervisor.Most of the time,her
supervisor and some staff of RMPH give her a task to encode some files they need. She was able
to apply her knowledge in using various computer packages like Microsoft Word and Excel
which include their Daily Reports, DOH funds, Guarantee Letter. She can handle such kind of
situation. Through the theories which she have learned.
English (Business Communication)
The trainees supervisor is the Administrative Officer of RoxasMemorial Provincial
Hospital. She usually sets the meeting for every department.She always ask the traineeif she can
do a MEMO or letter for her and the trainee was blessed because she already know what to do.
Management
The subject served as a guide to the trainee that she must be responsible in dealing with
the staff and clients of the RMPH. And it also serves as a guide to manage her time and to finish
the assigned task to her.

Lack of Facilities
Facilities are sometimes not available in times of need.

Employees personal problems can cause destruction to their performance which affects
their efficiency in the work.

Delayed salary
Salary is always delayed or not on time and that causes disappointment on employees
that also results to inefficiency and loyalty to work.
Noise
Noise outside the office disturbs the work concentration of the employees.

Some employees are not computer literate especially those old ones.

Government Hospital
Roxas Memorial Provincial Hospital is a public hospital so as people prefer to go there
than in other private hospitals.

Participation of local leaders to every activity of RMPH is one way to build confidence
among the people and the community.

Government Rules/Policies
Changing government rules and regulations may affect the operation of the hospital.
Administration may find hard to adjust with those changes in policies.

Political environment not favorable to the administration of the hospital can hamper the
development of the institution.
Negative Feedback

Negative Publicity due to clients complaints can cause a bad image of the hospital from
the community that may affect the trust and confidence of the public to the services of the
institution.

endeavor.This is one of the strength of Roxas Memorial Provincial Hospital. They have experts,
skilled, and experienced doctors with different field of specialization that can provide good
services to the patients.
Accesible location and geographic advantage
The location of the business or institution is considered as one of the assets of the
company because if the location is visible to the customers or clients it is not hard for them to go
to the office. The location of Roxas Memorial Provincial Hospital is visible to the people in
community. It is easy to have an access in this hospital because it is located at Arnaldo Boulevard,
Roxas City where it is considered as a center of all businesses in the city.
Government Health Care Provider
Government Health Care Provider is defined as a government owned institution whichis
composed of the doctors of medicine or osteopathy, podiatrist, dentist, chiropractor, clinical
psychologist, optometrist, nurse practitioner, nurse-midwife, or a clinical social worker who were
authorized by the state to practice and perform within the scope of their practice as defined by
State law, or a Christian Science practitioner. The Roxas Memorial Provincial Hospital is the only

government owned hospital located in Roxas City. The people will choose this hospital because it
provides healthcare services with an affordable price.

WEAKNESSES
Employees absenteeism and tardiness that affects their commitment to work
Absenteeism and tardiness cause an undue burden for those employees who substitute for
absent employees. Each employee was expected to report to work promptly at the beginning of
the day and after lunch. If an employee finds it necessary to be absent from work, such absence
should be immediately reported to the supervisor. A record of tardiness and inexcusable absence
may interfere with promotion and may result in loss of pay or disciplinary action. It is considered
as one of the weaknesses of Roxas Memorial Provincial Hospital because some employees or
staffs especially the casual and contractual have the tardiness habit that results to poor services to
the clients.
Inadequate information about their institution
Inadequate information about the institution can harm the reputation of the hospital. In
business, it is very important to know the background of the company in order for the company to
gain the trust of the customers or clients. The Roxas Memorial Provincial Hospital is one of the
oldest hospital in the province of Capiz. Inadequate information is considered as one of the
weaknesses because it doesnt have enough information to provide to the people. The hospital
does not have a historical background because it doesnt have enough data.
Inadequate equipment and facilities

The Roxas Memorial Provincial Hospital is a hospital that caters the poor and indigent
people in the province of Capiz. One of the weaknesses of this hospital is the lack of facilities and
equipment like CT scanner, ultrasound, MRI machine, etc. that are useful for hospital operations.
If the patients need undergo such body tests they are referred to another private hospital. This
situation is difficult to the part of the patients. It results to poor services among the patients and
clients.

OPPORTUNITIES
Fortuitous Events
Fortuitous event is an unforeseen event that occurs by chance or accident from natural or
man-made forces over which an affected person has no control. A fortuitous event can have either
positive or negative consequences. As a healthcare provider fortuitous event occur like floods,
typhoons, and other calamities, because of this uncontrolled events, many people get harm. It is
considered as an opportunity to the institution because it increases the income of the hospital
through the increase of the patients admitted into it.
Availability of new technology to improve services
Technology today affects every single aspect of modern society. Availability of new
technology is a big opportunity to healthcare industries. For example, the widespread adoption of
electronic health cares costs but secures patients health and safety. In more and more healthcare
facilities, patients files are being kept in databases that can be accessed easier. Diagnostics
through technological innovation can be easier and more accurate. Nowadays, numerous method
of imaging allow for technicians and physicians to examine a patients anatomy without needing

invasive procedures to form a diagnosis. The development of better instrument and more
advanced technology is a big help in the health care industries.
Changes in population or need that demand for healthcare services
The effects of population growth were varied and vast the population grows, the
opportunities for quality, healthcare services increases because the rate of admitted pregnant
patients and the rate of senior citizens patients that really demand for healthcare services.
Outbreak of diseases
Disease outbreak is the occurrence of cases of disease in excess of what would normally
be expected in a defined community, geographical area or season. An outbreak may occur in a
restricted geographical area, or may extend over several countries. It may last for a few days or
weeks, or for several years. When an outbreak of diseases occur in a certain place it demand
healthcare services. For example, like the outbreak of Dengue, Ebola Virus, Zika Virus etc.

Hospital is a public hospital that is lack of facilities, machines and equipment that can satisfy the

needs of the patients. Lives of patients seeking specialized treatment are at risk as public hospitals
are lacking the necessary equipment for assisting such cases.

affairs; and the opportunity to register as a candidate, to campaign, to be elected and to hold
office at all levels of government.Political participation extends beyond parties. Individuals can
also be involved in certain aspects of the electoral process through independent actionparticularly
at the local leveland by joining civil society organizations.
Roxas Memorial Provincial Hospital is a government owned hospital. It is powered by
people in politics. It is considered as a threat because if the governmental rules and regulations
will change, it willaffect the operations of the hospital.

which an affected person has no control. A fortuitous event can have be either positive or negative
consequences. Uncontrolled events are threats to all business establishments because it can affect
the business very much. For example, if there is a typhoon, fire, floods and other fortuitous

important files will be lost through this. It is another burden in the part of the institution because
it can result to higher costs for the repair of the damages in the hospital.

CONCLUSIONS
Based

on

the

SWOT

analysis

above,

the

following

conclusions

were

formulated:numerous numbers ofpatients admitted there every day but facilities were not enough

for their daily use and also the funds was not enough for the daily expenses of hospital.
Employees personal problem can destruct their efficiency of services, medical stuff is
incomplete, presence of new government regulation but the hospital cannotcope, and the political
intervention that influencing the system of the area.
RECOMMENDATIONS
Based on the conclusions above, the following recommendations were made:the area
needs topurchase such facilities and equipment to improve their systems, conduct seminars for all
employees particularly on stress and anger management,and upgrade the system in order to cope
with the new government regulation and political intervention. Orientthe employees to avoid
absenteeism. Take an immediate action to every complaint of the clients.

To start with my work as an OJT, Maam Lina Desales, Administrative


officer, assign me to her office. I also met Sir John Anthony Lim who is
kind and generous.

November 10

Maam Lina Arrives around 2pm and she asked me to file the Billing
Statement chronically.

Maam Lina Said that I should review the Book Service Based Office
Manual for me to knew those areas and responsibility and flowchart of
every department.

November 12

Maamlina give me hose Policies of every department that I need to revise


based on the book.

Encoded the Flowchart and Policies of every department.

I arranged the table of Maam Lina and also the Admitting files.

Encoded the Philhealth Policies and Flowchart.

Maam Roselyn the liaison of RMPH taught me how to record the


outgoing papers.

Wrote the outgoing papers in logbook.

Maam Gemma Assigned ma a task. I encoded the names of patient who


has a promissory note. After encoding Maam Gemma asked me to
printed-out the files I encoded.

We packed and wrapped the gifts pack for the patient of RMPH for the
gift giving.

December 4

I arrived early to arranged the gifts of patient and then we go to every


patient room by room with the heads of every department.

December 7-8

Once when Maam Lina was on leave, Sir Tam Taught me how to encode
a DTR and printed some of these for him.

December 9-10

I was assigned Maam Gemma to find the Financial Statement of the


patient name who has a promissory note.

December 11

I produced Financial Statement photocopies of those patient who has a


promissory note.

December 14-17

Maam Lina assigned me to do a meal ticket for the 585 employees of


RMPH, for their upcoming Chiristmass Party on Friday. After I print the
meal ticket I cut it one by one and arranged it alphabetically.

December 18

Christmas Party of all employees of RMPH and maam Lina Assimged


me at registration with Maam Gemma, to give the meal ticket.

December 21

We wrap all the raffle gifts for Christmas party of admin staff.

December 22

Christmas Party of Admin Staff at MVW and OJY is the incharged to


collect the gifts and their registration for the raffle draw.

January 4

Arranged the files and helped Maam Lina to clean her table.

Maam Lina Taught me how to solve the rate of IPCR of employees.

Maam Lina let me rate the IPCR and solve it.
