For and in consideration of the conditions and provisions specified in the Supply and Installation
Agreement, Jardine Schindler Elevator Corporation, hereby certifies that the Elevator Installation
Works is hereby completed in accordance with the contract, plans, specifications and contract
documents on the handover date stipulated above.
In compliance with your request for handover of the elevator and to be put to use, we have
reviewed and found the equipment to be completed with the best knowledge and information
and is declared to be functional and safe for public use on the date specified hereinabove.
The handover date shall be treated as the reckoning period for the warranty and free
maintenance provision of ____ 12 _______months on the completed Work which shall expire on
_________, 2012

*This Certificate shall be submitted as part of the requirements for the issuance of any and all government
licenses and permits and shall also be submitted as part of the requirements for the declaration on the
Public and Product Liability Insurance Cover that we will secure for the completed Work. The Public and
Product Liability Insurance Cover shall be enforced during the period of the maintenance works.
This Certificate shall be deemed accepted on the date above certified should the Client take possession of
the elevator and put into use despite failing to affix his conformity on the space provided herein, within
five (5) working days from the date of receipt.
