DIVISION AND REGIONAL PROCESSING CHECKLISTS TO EVALUATE/REVIEW DOCUMENTS


TO SUPPORT APPLICATION FOR PROVISIONAL PERMIT TO OFFER
SENIOR HIGH SCHOOL (SHS) PROGRAM

This Office is informing the field of the new Division and Regional Processing Checklists to use in
evaluating and reviewing the supporting documents of applications for Provisional Permit to Offer SHS
Program (Please see Enclosures 1 and 2), respectively.

Based from the suggestions of the participants of the Orientation cum Workshop for the Regional
Supervisors on the Senior High School (SHS) Program Documentation on May 27-29,2015 at TICC, Tagaytay
City, DepED Region VII proposed these checklists for the different Regions to adopt or adapt.

The Division Processing Checklist is used by any member of the Division SHS Task Force (to be
organized by the Division) in evaluating the appropriateness, correctness, and completeness of the
supporting documents submitted by school applicants, whether new applicants or existing applicants that
are updating their supporting documents.

Technical Assistance is given to schools if there are deficiencies to their supporting documents.
School applicants are given ample time to comply with lacking requirements. Note that ample
time is reasonable length of time agreed by both the member of SHS Task Force and the schoolapplicant depending on the requirement to comply with.
Applications are endorsed to the Region if supporting documents are found appropriate,
correct, and complete.
Applications may also be endorsed to the Region if some required supporting documents take
time to comply (i.e. Certificate of Occupancy).

To facilitate evaluation of SHS Program application documents, Schools Divisions must orient the
school-applicants in filling-out the following standard SHS application templates:

SHS Form 5: Class Program by Curriculum Offering (See Enclosure 6)

To check whether subjects in class programs reflect all the required SHS subjects (core, applied,
and specialized), the Curriculum Evaluator/Reviewer from the Division and Region must use the following
Curriculum Content Checklists (CCC):

To ensure compliance with requirements on materials, facilities, and equipment, Schools Divisions
must include in the orientation the requirement per curriculum offering. The list of these requirements can
be downloaded online from any of the following:

Enclosure 17 is a sample site floor plan of a basic education school. This serves as basis for a
comprehensive floor plan (by storey) to be submitted by schools applying permit to offer SHS Program.

Documents to support the applications to offer SHS Program must be arranged in order according
to the checklist.
9.
SHS applications endorsed to the Regional Office will be reviewed by the Regional SHS Task Force
(to be organized by the Region) using the Regional Processing Checklist.

If there are deficiencies to supporting documents, the Region will inform school-applicants
what documents to comply through the Division.
If supporting documents are found appropriate, correct, and complete, one or two members
of the Regional SHS Task Force together with a member of the Division SHS Task Force will
conduct a joint ocular inspection or on-site validation.

The Regional Director will issue the Provisional Permit to operate SHS Program to an applicantschool if all requirements are satisfactorily complied with and the findings of the ocular inspection is
commendable.

Schools previously issued with Provisional Permits to Operate SHS Programs are also encouraged
to take note of the teacher and materials and facilities requirement for each track/strand they offer to
ensure quality implementation of the program. If they want to offer a new track or strand or specialization,
all the items in the checklist have to be submitted.
