What Went Wrong

Althoughitisunknowntheexactcauseofthefailure,

turbowascapableofpossiblyproducedadestructive
rotationalresonanceoftheshaft.
Intenseheatratewithoutlettingmetalgraduallyincrease
Intense heat rate without letting metal gradually increase
intemperature.
Destructivefailureinwhichtheheatcausedthemetal
bladestoexpandandcomeincontactwiththeturbine
housingwalls.
