on your age, health and family history, and lifestyle choices such as what you eat, how active you are, and
so on.

CES Employees.

For appointments, the employee needs to give a call on the preferred centre number

Customer care executive will share appointment date and time.

It is mandatory for employees to carry the employee ID card also or any of their

The tests under the approved/pre-decided package are fixed. Any additional tests

Test

The employees needs to collect the medical test reports on completion of tests and
once medical test reports are ready at the concerned providers center.

The Health Checkup is open for the employees of CES.

Mode of payment

The Health check-up will be free for the employees of CES.

The employee may visit the hospitals to undergo screenings on any day subject to availability of
appointment.

Duration

11th July 2016 to 07th October 2016. (Office working hours with prior intimation of appointment)
Additional Benefits:

Family Member can also avail the offered above mentioned medical test with same package and
additional test on discount rates. Member needs to show Bajaj Allianz Health card for availing the
Health checkup benefit.

Family Member health checkup payment will be borne by the beneficiary as per the cost as
mentioned below-

Express Clinics- 25% on any additional test

BHIRC, Vadodara- 15% Discount on OPD investigations

Sunshine Global Hospital, Vadodara- 25% discount on Investigations.

Apollo Clinics, PAN India- 10% Discount on additional test as Corporate discount.
