4. PAYMENT AND OTHER FEES

 o Total contract price: Php __________________

o Down payment of 50% of the total contract price Php____________is required upon
signing of contract to officially block off the date. The remaining 50% balance will be
paid on the event day prior to the start of services. (You may consider accepting
downpayment lower than 50%, esp if weddings. From my inquiries sa makeup artists
for the wedding, 3k-5k lang dp.)

o Additional per head. Php _________ will be charged per head in excess of the
chosen package rate.

o Early start fee. If the Hair and Makeup team is required to start before 7:00 AM,
surcharge fee (+30% of the total contract price) will be applicable.

o Parking for the hair and makeup team. Client is to arrange for the venue(s)
parking for the hair and makeup team. If paid/metered parking fee applies, Client will
shoulder the corresponding fee.

o Acceptable payment term is cash basis only. (You can accept check but
effectivity will take place once check has cleared.)

5. OUT OF TOWN EVENTS

o For venues that may be reached for 2-3 hours land travel: +50% of the total
contact price.
o Destination weddings. Venues that may be reached by plane or more than 4
hours land travel is considered a destination wedding. The client will arrange for
and shoulder the fees that may include but not limited to transportation, parking
fee, hotel accommodation and meals (breakfast, lunch and dinner) of the Hair
and Makeup Team. (Once out of Metro ManilaOut of Town na. Dont base it sa
hrs of travel. Put premium fee na which is exclusive of transpo/hotel/food/parking
fee if applicable)

6. CANCELLATION

In the event that the Client cancels the project less than a week prior to the
scheduled date, the 50% down payment will not be refunded. The Makeup artist will
consider the deposit as cancellation fee payment.

(less than a week from scheduled date is too near. If you want, just give 10-15 days lead time
na pwede mag-cancel na refundable pa dp. Beyond that, non-refundable na. Pero with the
HMUAs na nag-inquire ako for the wedding, non-refundable talaga agad ung dp, walang lead
time.)
 Any no-shows on the appointment date will be charged the full price of services
scheduled. Client will have no further liability to the Makeup artist hereunder.

(No-show on the part of whom, you or the client? If yours, just say na if ikaw ung di
nakapunta for reasons beyond your control (use force majeure), you will refund all
payments made. If sa client naman, mahirap na habulin pa yan for full payment. Just
repeat na ung binayad na dp will be treated as cancellation fee)

7. PHOTO WAIVER

I, Client (FULL NAME) _____________________________ hereby authorize,

Katrina Orbista to use professional photos taken during the event as promotional
materials, provided photographer is credited accordingly. Photos from the event day may
be posted on Makeup by Kat Orbista websites, Social Media accounts, flyers/brochures
and other promotional materials.

8. LIABILITY

Makeup/hair Artist(s) shall provide services with reasonable care and skills. Artist
shall perform the services promptly on the date and time agreed. All makeup brushes,
makeup products and hair tools are kept sanitary and are sanitized between every client
requiring services. Makeup products used are of high quality, hypoallergenic and safe for
sensitive skin. Any skin conditions should be reported by the client to the artist prior to
application, and if need be, a sample test of makeup may be performed on the skin to
test any adverse reaction. Client(s) agree to release the artist(s) from liability for any skin
complications due to allergic reactions, which are not disclosed beforehand.

Makeup Artists obligation to give services hereunder is subject to the unavailability of

the Artist as a result of sickness, accidents, acts of God and other reasons beyond the
Artists control.

SPECIAL INSTRUCTIONS FOR THE EVENT

The bride/client should be ready (done with bathing, eating meals, toothbrush, etc.) prior
to the appointment time. Any delays caused by the bride/client will release the Makeup
Artist from any liability, the aforementioned delay may cause.
Hair should be freshly shampooed, dried and product free. No conditioner or any leave-
on serum should be used on the event day. Blow drying service not available, all styles
are done on dried hair
Face should be completely clean and bare. Absolutely no moisturizer, no acne creams,
etc. on the face.
Wear a robe or button down shirt.
