JOINT AFFIDAVIT OF TWO

DISINTERESTED PERSONS

WE, GLICERIO C. IRAN and TECLA C. BANABANA, both widowed, both

of legal age, both Filipinos, and both are residents of Tubtubon, Sibulan, Negros Oriental,
after having been sworn in accordance with the law hereby depose and state that;

We personally know JUANITA L. RETES, a legitimate daughter of the

spouses EMELIO E. RETES and SOFIA V. LIMBAGA, for we are long time
acquaintances and her family is a close friend and neighbor;

JUANITA L. RETES was born on January 31, 1956 at San Miguel, Sta. Catalina,
Negros Oriental and we know this fact because we are present during the delivery being
their close family friend;

We are not in any way related to any of the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have any pecuniary interest in this
matter whatsoever;

Upon her birth, she was not registered in the Office of the Municipal Civil Registrar
because of negligence;

We are executing this affidavit to attest to the truthfulness of the foregoing

statements and to establish the accurate date of birth of the abovementioned person to
support her application for late registration and for all legal intents and purposes it may
serve.

IN WITNESS WHEREOF, we have hereunto affix our signatures this 1st day of
March, 2017 at Sibulan, Negros Oriental, Philippines.

competent proof of their identities and that they executed this instrument
freely and voluntarily.
