age, Filipino, and residents of San Sebastian Village, Tarlac City and Brgy. San
Nicolas, Tarlac City respectively after having been duly sworn in accordance with law,
do hereby depose and state that:

1. We personally know the FRANCISCO DADULLA PADOLLO who is a

resident of San Sebastian Village, Tarlac City;

2. I, Irineo, know Francisco because the latter is my neighbor since 1967

while I, Rosita, know Francisco because the latter is a co-employee of
my husband at US Lumber since 1986;

3. We personally know that Franciscos birthday is on August 15, 1950;

4. This affidavit is executed without the taint of any improper or undue

pressure and influence exerted upon our mind and body.

5. We are executing this Affidavit to attest and affirm the truth of the
foregoing allegations and for whatever legal purpose it may serve.

neighbor from the time he was born;

2. In the PPSTA-MAS policy of TERESITA V. ANTONIO of Mark Jansen

Jacob V. Antonio his birthday is 09/02/99;

3. We know that the true birthday of Mark is on 08/02/99;

4. In view of the prevailing discrepancy, we were constrained to
