JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS

WE, NINDA L. GAMPON and ANGELITA Q. DE LEON, of legal

ages, Filipino citizens and residents of Barangay Bobon 1 st, Camiling,
Tarlac, under oath, do hereby depose and state THAT:

1. We both personally know NORDONIO ESTRADA GAVINO

(Nordonio, for brevity),who was born on January 29, 1943 to
Spouses Hilario Gavino and Felisa Estrada in Camiling, Tarlac;

2. Nordonio is a member of the Social Security System of the

Philippines (SSS) until his death on March 11, 2017;

3. When Mila Mendoza, the wife of Nordonio, went to the SSS

Camiling, Tarlac to file for the Death Claim Benefit of her late
husband, she was required to submit Nordonios birth, marriage
and death certificates or records;

4. After securing the required documents, it was discovered in

Nordonios Certification of marriage that his name was recorded
as NARDENIO GAVINO, and his father was recorded as HELARIO
GAVINO. Hence, the discrepancies;

5. We are not related to Nordonio either by consanguinity or

affinity, except that of being his barangay mates;

6. We are executing this affidavit to attest to the fact that the

names NORDONIO ESTRADA GAVINO, as recorded in the said
certificates of Live Birth and Death, and NARDENIO GAVINO, as
recorded in the said Certification of marriage, refer to one and
the same person, and that is our barangay mate Nordonio;

7. Also, we are executing this affidavit to attest to the fact that the
names HILARIO GAVINO, appearing as the father of Nordonio
Estrada Gavino in the said certificates of Live Birth and Death,
and HELARIO GAVINO, appearing as the father of Nardenio
Gavino in the said Certification of marriage, refer to one and the
same person, and that is the biological father of Nordonio;
 8. Finally, we are executing this Affidavit to attest to the truth of
the foregoing statements and for all legal intents and purposes it
may serve.

IN WITNESS WHEREOF, we have hereunto affixed our signatures
