Rating Justification

When doing my assessments I had forgot to check the patients heel and sacrum for skin breakdown. This is important to prevent the patient from acquiring any ulcers.

Opportunities for improvement

On my next assessment I will make sure to assess patient for skin break down. This can be accomplished by reviewing the checklist of assessment components before entering the
room.

I was able to maintain a therapeutic relationship with the client, this was accomplished by thoroughly explaining what medication she was to take and the effects of the drug. I also

expressed that I will answer any questions that she had. I also explained to the patient my experience with medication that she was using, and the side effects that had occurred.

Opportunities for improvement

Get a more precis evaluation on the mobility status of the patient by actively communicating about what the patient is capable of doing with assist or by herself.

Rating Justification

When doing my assessment I noticed that the patient had a red color on her leg. I didnt realize at first that this was part of the aseptic technique that the surgeon used before surgery
that gave it that color. I had to double check with my clinical instructor to make sure that it was actually an aseptic technique and not a problem with her leg. I eventually washed off
the excess on her leg.

When transferring the patient from the bed to the chair I had ensured that she had her shoes on to prevent falls. When responding to a call bell a patients IV pump was beeping. I
ensured everything was clear of occlusion and started it up again.

Opportunities for improvement

Ensure better instructions for patients transferring from the bed to the chair. This can include better motivational language to encourage the patient that she is progressing well.

Rating Justification

When providing AM care with a patient, I causally explained the importance of calf pumping to prevent blood clotting. The patient was very receptive to the advice and seemed to
