In accordance with HFAP regulations (04.00.11-04.00.12), orientation and annual re-training must be provided
to all categories of personnel, as well as the Medical Staff and any contracted staff. This packet contains an
overview of the eight mandatory topics set forth by HFAP, as well as a short evaluation designed to document
knowledge acquisition. This post-test must be completed and submitted to the Medical Staff Office.
