A.D.C. : Making effective decisions to match with client needs involves considering three factors of

equal importance: the client, the nurse and the environment, and deliberating on how they apply to the situation. I am also learning to be more aware

to situations, where I have to be more prepared such as remembering to wear proper PPE in isolation rooms and to not spread infections from patient

to patient. I am more careful now and I always keep in mind to do proper hand hygiene, how to use proper body mechanics and very careful when

moving patients, because I am accountable and responsible for whats best for my client.

my lectures, notes and my teachers. when I am not sure about something I always make sure to look for answers first before I come forward working

on my way. I do not feel comfortable working on something that I am not sure of, therefore I always make sure that I always come prepared for work.

A.D.C. : I always make sure that I demonstrate my understanding about my role as a nurse within the health care team. for example by making sure

the medications of a patient is safe to give by collecting data and making sure they are in the right parameters. another example is trying to

A.D.C. : During post conference in our placements, I always make sure I participate in any group discussions and sharings to have a broader

understanding to other concepts of nursing and to also know what other situations can happen from the stories and views of my other classmates. not

only that I am also learning to to my best using the SBAR when discussing patients to other healthcare providers in the team.

A.D.C. : I always document everything right away when i do assessments to my patients by using an electronic method. I always make sure that they

are always right on time, to make sure that the plan of care is done in a timely manner. it is always the best to document as soon as possible so that

when there is something unusual to the patient, it can be resolve as quick as possible or can be determine right away.

A.D.C. : I always report revelant information to my preceptor firsts and as well as the other nurses working on the floor when there is a problem on
the floor or with the patients before taking an action especially if I'm not sure of. when there is something unusual with my patient I aways make sure i

come on contact with the patient's primary nurse before taking an action, because first of all I do not have all the answers, and second the primary

nurse sometimes knows a lot about the patient, because they were been the primary nurse of the patient before.

A.D.C. : I aways try to give my client's to speak up during their care, and tell me what they want to provide respect, and to provide a great client nurse

relationship as well. Having a good communication with the right people in the team and with the patient will provide a good client-centered care and it

will make sure that the client's choices are met. Communicating with the family is also done to promote health teaching and to make sure that they are

also aware to the patient's plan of care.

A.D.C. : I will always keep in mind to give some privacy and confidentiality for my patients, during their care especially during their morning care

when they are all naked while getting dressed. I always keep in mind that my patients are the same person like me, who gets embarrassed and have

feelings to deal with when they wanted to hide something from themselves. I always try to think about their feelings if that would have happen to me to

give their dignities and to give respect.

and my other handy books to follow up some of the uncertain things based on the nursing topics I have. but I should always keep in mind to research

A.D.C. : From working in the hospital from the beginning till now, I have found myself improved in my communication skills. by having a primary nurse

taking care for my other patients, I also have learned to use the SBAR in the facility at some points, when talking to nurses, occupational therapists,

speech therapists and doctors. it is a bit overwhelming but good practice will make me better at it.

A.D.C. : In my first few weeks in the hospital I have demonstrated therapeutic care for my patients, I am compassionate about my work and I tried to

be as culturally aware with my clients and to health care team members, so that I can have a great relationship with them. By doing this, I can create a

great bond with the team and as well as have a good nurse-client relationship. Until now I am still therapeutically caring compassionate and culturally

safe with my patients while I provide care and treatment.

A.D.C. : I use self awareness to the effects that beliefs, values and personal experiences in relation to my practice. Being self aware is going to

provide great work for my practice and I will be able to provide a better nurse client relationships, because if you are not self aware it my effect your

work as a nurse. knowing my strengths and weakness in the beginning will make me a better nurse so I can improve more to the things I need to

improve on.

A.D.C. : I have not consulted to any of the inter-professional health care team yet but If there is a situation where it is in need to consult I will try to

collaborate with the members of the health team by using the SBAR and to make sure to do it appropriately. I do somehow did use it in times where

nurses, and other care providers ask me questions about an specific patient; But by doing this I also have to make sure that the person who I am

sharing the information to is the proper personnel, because I do not want to break the privacy and confidentiality.

A.D.C. : I always support the diversity of any of my clients and the inter-professional health care team, because as a team we have to support each

other to make the care plan work for us and especially for the patient. Sometimes support is the only thing you can do to specific patients depending
on their situations because there are time where they cannot be cured or cannot go back to their old lives anymore, that is why supporting your clients

is very important.

A.D.C. : To provide client-centred care I always try to incorporate my client's wishes with their care because I want them to feel comfortable while

they are being taking cared for. From the CNO guidelines I have to value their wishes and decisions with their care to provide client-centred care.

From this I can build trust between me and the client and have a good nurse-client relationship, but there are also limitations that I have to follow.

A.D.C. : During my care for my patient, from assessment to evaluatation I always provide some client education according to their health and
treatment. Providing client education by explaining them simple and understandable meanings of their treatment will somehow make them more

cooperative with their treatment. That's is why it is very important to provide them information on how to do a procedure, on how to eat well and etc.

When the patient's family is in the room make sure that you talk to them as well and let the know what is happening and as well educate them so that

the stress and feeling worried to their love ones lessen.

A.D.C. : Whenever there is conflict in situations I always make sure that I seek for guidance first especially to the situations that I am not sure of.

However in the future, I should be able to strategize because I can't always depend on my peers and preceptor in my career, therefore as a novice
nurse I will seek guidance first before acting.

A.D.C. : I am not really a good leader who leads, but I always make time to help out my peers and work out things together whenever something is

not looking good. By doing this it can also help me improve my communication skills and can create a positive working environment.

A.D.C. : During sim labs and post conferences I always try to participate and give out feedback a with my fellow nursing students and accept

feedback from my preceptors, health care team members and peers to help me improve my skills. I use this feedback to see my weakness and help
me improve them. We also have done some peer feedback forms given out to our peers to provide them what their strengths and weakness are, from

this it can provide them some information on what to develop on to further their skills in nursing.

A.D.C. : I always advocate for my clients, self and others and the quality environment. by doing so, this will create a positive learning environment for

A.D.C. : I always try to incorporate and support clients needs and choices to help them out with their therapy. Because it is very tough for a client to

try and become healthy if their needs are not met especially if it doesn't get some good support. By providing support to your client it will also give a

A.D.C. : Whenever I encounter some unsafe, unacceptable and unprofessional behaviours I always seek for guidance and support or let someone

know about it right away, so that i can prevent it in the future. If I have witness something that is unsafe, unacceptable or and unprofessional, I make

sure that I will confront that person first before making assumptions.

A.D.C. : I make sure that I always collaborate with the health care teams for any nursing practices for the public and also for my clients as well. By

using collaboration it will help nurses to figure out what types or interventions and medications the can provide to the patient. it also helps to see how
the patient is responding to the treatment and care, if he or she is getting improvement or not.

solutions and strategies by finding strategies what's best for the patient first, working with the team and talk about the strategy that has been used if

we can keep using it or not.

A.D.C. : Before my shift starts, I always make sure I come in on time to get the night shift reports from the night shift nurses and because of this

collaboration it will help me to determine the needs of my client and achieve the goals that is set for that day. Organization and working collaboratively

with the health care team is really an asset because as a nurse you can not do it all alone.

A.D.C. : As a team member of the inter-professionla health care team I make sure I try helping the other nurses with their needs and reach goals for

being reach or if the care is working to improve the patient's state.

A.D.C. : teamwork is the key to help out clients to get their goals. One nurse can't handle all the job so collaboration with the health care team is very

A.D.C. : whenever the patient needs to consult with anyone in the heath care team due to their situation, I will try to help them arrange that according

to their schedule. I will have to talk to the therapist or who ever they need, so that they know some the patient's baselines and health histories, so we

can work together and meet the client's goals.

A.D.C. : As a team member I want to get treated the same way I treat others and that is the same to the other team members. I respect any of the

team members and treat them equally to have a nice working environment for us and also for the clients. PSW's, senior nurses, doctors, and other

care providers are all the same people and they needed to be treated in a respecting manner even though we have different roles and even though

our levels are not the same, because we are a team.

A.D.C. : I use nursing research to foster my professional development, such as going to the CNO practice guidelines online and reading case studies

that can happen to me in the future so that I have at least have an Idea on what to do in a situation that I haven't encountered from just reading.

A.D.C. : I always use my theory based approach and evidence-informed practice by organizing my work. By using my books, my knowledge, skills,

judgement and the help of my teachers helps me to accomplish everything in my practice. I always make sure to incorporate everything that I have

learned since day 1, from the basics till now; by using assessments, checking on lab works and making sure they are all incorporated together,

knowing all the normals and abnormals so that you can recognize unusual things. these approaches are all important to see everything as a whole in
the nursing career and that is what use in my practice.

these affect nursing practice, and I feel that I have improved my thinking based on the situations i have every clinical.

A.D.C. : I have found myself improved using my critical thinking and problem-solving skills to decision-making in my clinical placement. whenever I
am in clinical i have been finding ways and solutions to improve my care plan for my patients, on how the schedule of their day going to be like, how

the medications will effect their day how and how will it affect them. I also have found myself improve working with two patients since after midterm.

A.D.C. : Like said from above, I also have found myself improve on critical thinking, problem solving and decision making skills when assessing

clients. I found that when I make concept maps every week I, it really helps to determine what priorities i have in each nursing diagnoses. it helps me

to figure out what are my priority interventions and helps me to plan my day out by knowing what should be done first according to my patient's

diagnoses.

A.D.C. : my critical thinking, problem solving and decision making has improved throughout my clinical experience. in the beginning I thought that I

starting in this hospital.

weeks. every time I get different patients, they usually get a variety of nursing assessments and interventions, these interventions and assessment

have to be prioritized because you can not do all off it at once. by having different patients I have learned to prioritize what comes first and whats best

first for each of my patients.

everything before I start caring for may patients, and every plan has to come up with a decision. the decision that I come up with always depends on

the patient's diagnosis, mental and physical health, and environmental factors, So I make sure to make my decisions according to these factors to

make the care and treatment the best I can.

Overall, since Day one working in the nephrology unit I have seen myself grow, I have learned so many things compared to when I was in the long
term care facility. in the beginning I even thought that I have not going to survive this semester, but I have, I have finished it. I learned a lot of of
nursing skills, Learned a lot more medications, I have learned to communicate better by using my SBAR, I have learned how to prioritize and make
decisions in different situations, I have been exposed, not a lot but just enough.I know that there is a lot more space for continuation of growth, my
adventure has not finished yet and it will still continue. I believe that in my next clinical placement, I have to make sure to always practice handling
more patients, keep doing concept maps, keep practicing on my organization skills, critical thinking skills and decision making skills. I will try to be
more prepared, I will review everything all over again so I can do the best that I can, because I want to be the best nurse that I can.
