Healthy People 2020 Goal

-In Healthy People 2020, a goal states Improve access to comprehensive, quality health care

services (Access to Health Services, 2014).

Short Term Goal

-To provide education within the community pertaining to available resources in order to make

community members more aware to resources that are available in close proximity, if not within

the community.

Long Term Goal

-To establish a community based inpatient and outpatient healthcare facility or urgent care center

in the city of Struthers.

Outcome Criteria

-Within three months, the city council will get together to discuss the establishment of an

inpatient and outpatient urgent health care facility. The meeting will take place in city council

hall to determine the likelihood of funding for the facility.

-Within six months, the funding for the healthcare facility will be established, the construction

blueprints will be in development, and the construction date and location will be selected.

-Within eighteen months, the healthcare facility will be established, staff will be hired, and

appropriate physicians will be implemented. An inpatient screening process will be put together

and a grand opening day will be set.

-Within twenty-four months, the Struthers Community Health Facility will be open to the public,

referrals to the facility will have been implemented and residents of Struthers will be seen and

treated for the health issues they have.

Nursing Diagnosis

-Risk for ineffective health management for the people of the city of Struthers, related to the lack

of inpatient and outpatient healthcare facilities and insufficient financial resources

As Evidenced By:

-The city of Struthers lacks any hospitals, urgent-care facilities, and only has a few physicians

located in the city (Preventative Care, 2016).

-Many of the citizens of Struthers must travel to Youngstown or Boardman to receive healthcare,

as the only nearby hospitals are located in these cities (Preventative Care, 2016).

-More than 13% of the population of the city does not have healthcare insurance (Preventative

Care, 2016).

-A proportion of individuals falling below the poverty line that is higher than the national

average (Preventative Care, 2016).

Interventions

1. Assess for indications that the community may be unable to effectively manage

healthcare.
2. Provide the community with written instructions about scheduled appointments with

health care providers, medications, signs and symptoms to report, weighing selves, and

dietary modifications.
3. Encourage community members to openly discuss concerns about the cost of

hospitalization, medications, and lifelong follow-up care.


4. Reinforce the fact that social service consults to assist with financial planning and to

obtain financial aid are available.


5. Provide information about and encourage utilization of community resources that can

assist community members to make necessary lifestyle changes.

1. Before aid can be given to prevent ineffective management of healthcare, indications of

ineffective healthcare must first be identified (NURSING DIAGNOSIS: Ineffective

management, n.d.).
2. By learning the importance of taking measures to improve the quality of life community

members will be better equipped to make better decisions regarding their health in order

to prevent ineffective health management (NURSING DIAGNOSIS: Ineffective

management, n.d.).
3. To help identify factors that may put the community and its members at risk for

ineffective health management so that they may be addressed appropriately; open

discussion is a healthy and effective way to voice concerns (NURSING DIAGNOSIS:

Ineffective management, n.d.).


4. Some community members that are at risk for ineffective health management for

financial reasons may not be aware of social services that may help them acquire the

level of care that they need and deserve; seeking social services may help to increase the

likelihood that community members will seek healthcare (NURSING DIAGNOSIS:

Ineffective management, n.d.).


5. Some community members that are at risk for ineffective health management may not be

aware of community resources available to everyone such as health clinics. By

increasing knowledge of such programs community members will be more likely to seek

them out in order to improve community health as a whole (NURSING DIAGNOSIS:

Ineffective management, n.d.).

Evaluation:

After three months, members of the community established the council and meetings were held

to discuss the need for a healthcare facility. The council was awarded a community facilities

grant from the state of Ohio and the United States Department of Agriculture Rural Development

to assist in funding the healthcare facility.

Six-Month Evaluation

After six months, the full amount of grant funds from the state of Ohio and the United States

Department of Agriculture Rural Development were received. Funds were used to hire

construction personnel, acquire equipment, and purchase supplies. Engineer and architectural

team completed blueprints successfully. Beginning of construction date was chosen; end-date of

construction is approximately 365 days. Location was selected on 5th street; Struthers, Ohio.

Eighteen-Month Evaluation

After eighteen months, the facility was erected without any delays. Three physicians have been

acquired to oversee patients. Twenty BSN level registered nurses have been hired to provide

assistance to the physicians and treat medical conditions. Other staff have been hired to maintain

day to day functionality of the facility (e.g., sanitation and administrative support). The facility

has on-site X-ray, EKG, and laboratory. Finalization of necessary legal documentations and

background checks near completion. Grand opening scheduled for six months barring any

delays.

Twenty-four Month Evaluation

After twenty-four months; Struthers Community Health Facility opened its doors to the public

for urgent care. The Struthers Community Health Facility offers 20 beds for inpatient care and

10 beds for acute outpatient care. The facility offers treatments for allergic reactions, colds,

cough/bronchitis, infections, flu, minor fractures, sprains, vaccinations, and physical exams. The

facility census has been high; providing safe and effective healthcare to the residents of Struthers

and the surrounding communities.
