Position Details:
Position Title: PHaM Case Worker
Division: Community Services
Reports to: Program Manager
Position Purpose:
To work with individuals who have a severe functional limitation resulting
from their mental illness and assist them to overcome social isolation and
increase their connections to their community. This will require building
relationships to enable referrals and close working relationships with the
Government and non-Government sector with particular attention to the
allied health sector.

Recruitment information
Qualification, knowledge, skills and experience required to do the role
Tertiary qualifications in the Allied Health field or significant demonstrated experience in a
similar role
Minimum 3 years Case Management experience
Demonstrated experience Working with Aboriginal and Torres Strait Islander communities
Computer skills in Microsoft Office, and ability to adopt Mission Australias web-based
Information Managements System (MACSIMS training will be provided)
Current valid drivers license
