I. Identify ( 20 pts)
Background 1. These accident factors form the unsafe behavior pattern displayed by the
factors person within a given unsafe environment.
Initiating
factors 2. These accident factors involve persons deviating from normal pattern or
standard procedures coupled with some catalyst for accident.
Intermediate 3. These factors emphasizes on the condition wherein the individual
factors continues with the accident-producing behavior pattern.
Immediate
factors 4. This factor is considered the trigger mechanism wherein the individual
commit an unsafe act that will certainly cause an accident.
Mitigating
factors 5. This factor represents possibility that might develop at the last moment to
eliminate accident about to happen.
85 percent 6. The percentage contribution of unsafe behavior of an individual with
respect to all accident.
15 percent 7. The percentage contribution of unsafe environment with respect to all
accident.
accident 8. A perfectly natural product of an unorganized way of life.
Philosophy
of life 9. An experience wherein an individual was about to encounter due to unsafe
act but at the last moment, no measurable results in terms of injuries or
damages occurred.
Safety 10. A condition or state of being resulting from the modification of human
behavior and/or designing of the physical environment to reduce the
possibility of hazards, thereby eliminating possible accidents.

II. ENUMERATE & EXPLAIN (60 pts)


A. Five classes of unsafe behavior (20 pts)

1. Physical limitations some accidents are caused by an individual


attempting to perform acts for which has lack of capability to be done with
their physical condition

2. Knowledge lack of knowledge often be the cause of accidents


performing acts that doesnt have thorough understanding with performing
the task

3. Skill skill is important to avoid accident that every person performing a


task must have proper training which leads to safety and perfection

4. Habits it is important that task is regularly checked to avoid the incorrect


habits repeatedly done and may cause accident to develop good habits
starting from the beginning

5. Attitudes it is important to have good attitude especially from the


persons doing the task in a long period of time because it can cause
accident because of not doing the safety precautions anymore because of
the fact that they already familiarizes the actions and they are more
confident on doing actions in their own ways.
B. Five factors causing an accident (20 pts)

1. Background factors these factors form the behavior pattern by the person
within a given environment

2. Initiating factors for an accident to occur, these must involve the person
on a particular day and a specific activity

3. Intermediate factors these factors tend to increase the susceptibility of


the individual and also enhance the accident potential

4. Immediate factors commits an unsafe act and when such act is


performed, the person has gone to the point of no return

5. Mitigating factors there is a possibility that something might develop at


the last momet to dispose of the accident in the making

C. Five Personal Protective Equipment (20 pts)

1. Safety shoes is a durable boot or shoe that has a protective reinforcement


in the toe, usually combined with a sole plate, which protects the foot from
falling objects and punctures from below.

2. Safety helmet A helmet is a form of protective gear worn on the head to


protect it from injuries.

3. Safety gloves Protective hand covers that reduce the risk of injury and
exposure to high temperature substances or toxic chemicals.

4. Safety goggles are often worn when using power tools such
as drills or chainsaws to prevent flying particles from damaging the eyes.

5. Safety harness some safety harnesses are used in combination with


a shock absorber, which is used to regulate deceleration when the end of
the rope is reached.
III. ESSAY (20 pts)
A. List down your insights and learning from the statement written below.

ANSWER:

The statement explains the importance of safety being a part of our plans and
value structure in our daily living. Integration of safety must always be a part of a
persons philosophy of life thus it will be a part of our controlled behavior that may
contribute to our society for an individual. Integration of safety helps an individual to
build a better value structure to integrate accident prevention procedures to our daily
activities and making it a habit.
