INTRODUCTION
Cervical cancer is a cancer that is present in the cervix or uterus, which is the
area that connects the bottom of the uterus uterus with the vagina. (Emilia, 2010). In
2003, WHO declared that cancer is a very serious health problem because of the
number of patients increased by about 20% per year. Breast cancer is the second in
Indonesia types of cancer affecting women after cervical cancer (cervix). In other
words, the first order cervical cancer is the most affecting women in Indonesia.
(Azamris, 2006).

Around the world, cases of cervical cancer has been experienced by 1.4
million women. The data obtained from the World Health Organization (WHO) finds
that there are 493 243 inhabitants per year of new cervical cancer patients with a
mortality rate as much as 273 505 inhabitants per year. (Emilia, 2010). Until now,
cervical cancer remains a health problem for women in Indonesia in connection with
the incidence and mortality from cervical cancer is high. Delay in diagnosis at an
advanced stage, the general state of the weak, low socioeconomic status, limited
resources, lack of facilities and infrastructure, histopathological types and degrees of
education to participate in determining the prognosis of patients. (Rasjidi, 2007).

In Vietnam, cervical cancer is the first cause of death of women while in

Indonesia and the Philippines, cervical cancer ranks second leading cause of death in
women, while in Thailand and Malaysia, cervical cancer occupies the third leading
cause of death of women. In Indonesia alone, an estimated 15,000 new cases of
cervical cancer occur each year, while the number of deaths estimated 7,500 cases per
year. Every day is estimated to occur 41 new cases of cervical cancer and 20 women
died of the disease. In 2009, new cases of cervical cancer amounted to 2,429, or about
25.91% of all cancers found in Indonesia. With the incidence of this, cervical cancer
is second only to breast cancer in women of childbearing age 15 -44 years. (Wijaya,
2010). The average age of the incidence of cervical cancer is 52tahun, and the
distribution of cases reached the summit two times at the age of 35-39 years and 60-
64 years.

1
Cancer of the cervix itself is a malignancy that can be prevented due to:

1. Have a period of pre-invasive (before becoming a malignancy) long

2. Cytology (cell) for early detection of cervical cancer have been available
3. Therapeutic pre-invasive lesions (seed malignancy) is quite effective
The results showed that the problem of cancer in Indonesia, among others,
nearly 70% of patients with this disease were found in the advanced stages. The
highest tumor prevalence by province Daerah Istimewa Yogyakarta is at 9.66% and
the lowest was 1.95% North Maluku. While the order of the type of cancer or tumor
Indonesia is the highest in ovarian cancer and cervix uteri. (Oemiati, 2011).

Visual Inspection with Acetic Acid (VIA) is a visual examination of the cervix
using acetic acid to the naked eye to detect abnormalities after application of 3-5%
acetic acid (MOH, 2009). Based on research conducted by Sankaranarayanan, et.
altentang comparison of patients with cervical cancer who died in the group that
performed 3 early detection by IVA and the group that did not do early detection of
developing countries (India) showed that those who have been screened IVA, 35%
less likely to die than those who do not get screening IVA.

The majority of women diagnosed with cervical cancer usually do not do early
detection (screening) or do not follow up after it was discovered the abnormal results.
No early detection regularly are the biggest factors causing the outbreak of cervical
cancer in a woman, especially because it has not become a mandatory program of
health services. (Emilia, 2010). Scope of examination for early detection of cervical
cancer using the Pap felt not to include the goals. Currently IVA early detection
method is a recommended practice for facilities with low resources compared with
other types of screening. When combined with the Pap smear, visual inspection after
the cervix is swabbed with acetic acid for one minute increase detection to 30%.
Study in South Africa found that the IVA will detect more than 65% of early lesions
and invasive cancer that investigators recommended as alternatifskrining cytology.
For comparison, in Zimbabwe screening IVA by midwives had a sensitivity of 77%
and a specificity of 64%, while the Pap smear has a sensitivity of 43% and a
specificity of 91% . According these results, it can be seen that the sensitivity IVA
better despite lower specificity. ( Emilia, 2010).

2
Data from 2007 show that the visual examination of the cervix using acetic
acid (VIA) at least equal efficacy with the pap smear test to detect the disease and can
be done by a trained midwife and with fewer logistical and technical barriers, low cost
and can be done to control cancer the cervix with the facilities and resources are
limited. In developing countries such as Indonesia where resources are limited, the
methods of IVA (Visual Inspection with Acetic Acid) are most suitable to be applied
as a screening method for cervical cancer. (MOH, 2009).

All women are at risk for developing cervical cancer. However, several risk
factors that increase the chances of cervical cancer include age, women aged 35-50
years are still sexually active and vulnerable to cervical cancer. The age of first
intercourse is also a risk factor for cervical cancer, about 20% of cervical cancers
found in women who are sexually active before the age of 16 years. Number of sexual
partners contribute to the spread of cervical cancer, the greater number of sexual
partners that also increase the risk of cervical cancer in women. The frequency of
pregnancy also increases the risk of cervical cancer because of a history of infection
in the genital area. Women who smoke or second-hand smoke also increases the risk
of cervical cancer. Besides the use of the contraceptive pill in the time period
lamajuga increase the risk of cervical cancer. (Wijaya, 2010).
