AFFIDAVIT OF WITNESS

I, CATHERINE HANG, 25, single, resident of 143-C Modesto

Pumapag-ibig Street, Manila, Filipino citizen, after having sworn in
accordance with law, do hereby depose and state THAT:

1) I was present and personally saw that defendant, ASTER

CARAMEL GIRL CARILLOentered into a contract of lease with
plaintiff, ZESYL AVIGAIL FRANCISCO-ANGELES;

2) The defendant is the lessee of the plaintiff and the latter is the lessor
of the leased premises located at 143-C Modesto Pumapag-ibig
Street, Manila;

3) The defendant signed the contract of lease on February 10, 2014;

4) I personally witnessed and signed the contract of lease as a witness,

on the same date, February 10, 2014.

I executed this affidavit to attest the truthfulness of the foregoing

facts and to support the filing of complaint for Unlawful Detainer against
ASTER CARAMEL GIRL CARILLO.

Street, Manila;

AFFIDAVIT OF WITNESS

I, ZESYL AVIGAIL FRANCISCO-ANGELES,27, married,

resident of 143 Pumapag-ibig Street, Manila, Filipino citizen, after having
sworn in accordance with law, do hereby depose and state THAT:

1) I together with my husband owned the leased premises located at

143-C Modesto Pumapag-ibig Street, Manila;

2) I entered and executed a contract of lease with the defendant,

ASTER CARAMEL GIRL CARILLO, as the lessee, starting
February 14, 2014 up to July 14, 2015;

3) The defendant, as provided for by the lease contract agreed to pay a

monthly rental rate of Five Thousand Pesos (P5,000.00) to the lessor;

4) The contract of lease has already expired last July 14, 2015, but the
defendant did not inform me that he had any intention to renew or
extend the lease contract;

5) The defendant refused to talk to me and also failed to pay monthly

rental starting July 15, 2015 up to the present;

6) Despite my continuous reminder and request for him to pay the

monthly rentals or vacate the premises, he still refused to pay;

7) On May 2016, I already seeked the advice of a lawyer and was adviced
to send a letter of demand to the defendant;

8) I sent my first letter of demand to the defendant on August 2, 2016,

but the defendant continuously evades me, by not responding to my
messages or to my calls;

9) I sent my second demand letter on August 24, 2016 as adviced by my

lawyer;

10) Due to the lack of response by the defendant, I was forced to

send my third and final letter of demand dated October 14, 2016, and
by reason of defendants failure to pay the monthly rentals and/or
vacate the leased premises, I was constrained file a case against the
defendant.

I executed this affidavit to attest the truthfulness of the foregoing

facts and to support the filing of Unlawful Detainer against ASTER
CARAMEL GIRL CARILLO.
