I have a sincere interest in your current posted position because I have a true passion for helping our
community and bridging the gaps within the healthcare system to provide patients with appropriate access
to care and services. I possess extensive experience in the healthcare industry and am looking for a position
where I can take an active role in changing health related behaviors by promote resources that lead to
healthier lifestyles and behaviors. I am a strong supporter and advocate of outreach efforts and providing
the necessary resources available to those in need of healing. I am passionate about enhancing the quality of
life of those who suffer from conditions as well as assessing the existing mental health issues that lead to
problematic health behaviors. I recently became a certified Community Health Worker and certified
leader/facilitator of the Stanford University School of Medicines Chronic Disease and Diabetes Self-
Management Programs through Dignity Health. I have a true passion for helping others and have a deep
interest in working with organizations like yours who seek to enhance the patient experience by taking a
whole approach to health, not simply focusing on a specific disease state but rather taking an approach
that looks at all existing variables that effect wellness.

I possess nearly ten years of experience in the hospital/healthcare industry with community outreach efforts
related to wellness being an integral part of my role. My current and previous work roles have provided me
with a great deal of exposure to the dynamics of working within various diverse populations and developing
curriculum tailored to the specific needs of each sub group. In addition, my experience as a leader/facilitator
or various programs has proven the effectiveness of supportive programs that enhance quality of life, provide
social support and lead others through stages of change and life transitions. I firmly believe in life-changing
components of these programs as I have observed changes in participant lives and behaviors as a result.

I have a deep interest in working closely alongside patients and members of our community and being an
integral part of ensuring preventative and transitional techniques become learned behaviors, keeping
community members out of the hospital, avoiding readmissions and unscheduled physician visits. My recent
certification as community health worker has provided the skills necessary to advocate for patient needs and
serve as resource between the patient and various healthcare and community agencies. My previous
background has provided the community, physician and healthcare background necessary to ensure patients
are provided the absolute best care they deserve. I look forward to contributing to an organization with
