A lot had changed with the couple since my visit last year. Several
months ago, the wife fell and broke her shoulder. Her husband
had been so busy taking care of her, he had neglected his own
health.
He told me right away he had not been feeling well over the past
several days. He had diarrhea, neck and ear pain, tingling on the
right side of his head and often felt very lightheaded. My neuro
exam showed everything normal, but I found his blood pressure
quite high and his pulse unusually low. He was already taking
three blood pressure medications.
He balked at the notion of seeing a doctor. He didnt want to leave
his wife. But by the time I finished the assessment, he agreed.
I initially left a message, but his doctor called back directly
wanting to know more about the members situation, who I was,
the nature of the assessment, etc. With his questions answered,
the doctor advised the member to take an extra dosage of one
medication and to call back in a couple of hours. If the blood
pressure had not dropped sufficiently by that time he was to go to
the ER, otherwise an appointment was set for the next day.
I wrote down the doctors instructions and ensured the member
took the medication before leaving. I later called back to ensure
everything had been carried out. It had. His blood pressure
dropped sufficiently to where the ER was not needed and the
member committed to making his next-day appointment.
