MEMORANDUM OF AGREEMENT
KNOW ALL MEN BY THESE PRESENTS:
This agreement made and entered into by and between:
CONCEPCION HOLY CROSS COLLEGE, a non-stock, non-profit educational
institution, duly represented by its Managing Directress Dr. Raquel T. Sta. Ines,
with office address at Concepcion Holy Cross College, Barangay Rosepark,
Concepcion, Tarlac hereinafter referred to as the First Party;
-and-
CAPAS FIRE STATION, duly organized government agency with office address at
Capas, Tarlac and herein represented in this act FSINSP. RENE P. FESETAN, Municipal
Fire Marshal, Capas Fire Station, herein referred to as the SECOND PARTY;
WITNESSETH THAT:
WHEREAS, the First Party recognizes the limitations of theoretical or
classroom instruction in providing its BSBA Financial Management students
with the necessary training and development essential to their careers;
WHEREAS, the First Party is cognizant of the importance of supplementing
theoretical training and instruction with actual practice and hands-on experience;
WHEREAS, it is to the advantage of the graduating students of the First
Party to undergo onthejob training at the Bureau of Fire Protection such as the
Second Party and for which reason this Agreement is entered into;
WHEREAS, the First Party seeks the aid of the Second Party in helping it
provide its BSBA Financial Management Interns with a complete learning
experience so as to enhance their training and development as future law
enforcement authorities;
NOW, in view of the foregoing premises, the Concepcion Holy Cross College,
and the Bureau of Fire Protection ____________________ hereby agree on the
following terms and conditions.
A. COMMITMENTS
The First Party shall:
1. Be responsible for the overall management of its BSBA Financial
Management Interns undergoing their OntheJob Training (OJT);
2. Allocate funds for its onsite supervisor for inspection purposes;
3. Provide a practicum instructor to be in charge of its BSBA Financial
Management interns prior to and during the actual OJT;
4. Conduct inspection and monitoring of the trainees every Monday, anytime
from 8 am to 5 pm through its assigned coordinator;
5. Provide training materials such as kits, handouts, and other instructional
materials to its BSBA Financial Management Interns;
6. Monitor the compliance of the undertakings stated in this Agreement; and
7. At the start of the internship, provide the Performance Evaluation Sheet
(PES) to be accomplished by the Second Party.
The Second Party shall:
1. Provide technical assistance and instruction to the BSBA Financial
Management Interns for them to acquire the skills essential in the dayto
day operations of BFP
2. Train, expose to, and assist the Interns about the standard operating
procedure of the agency in financial management.
3. Facilitate the coordination between the interns and its security complement
for the duration of the internship;
4. Ensure the personal safety of the interns within the duty premises for the
duration of the internship;

5. Provide a certificate upon completion of the Three hundred (300) hours On-
The-Job-Training; and
6. Accomplish fully the Performance Evaluation Sheets (PES), folded and
sealed, to be provided by the First Party through the interns Team Leader.

B. PROVISIONS COMMON TO BOTH PARTIES


1. The internship shall be at no cost to either party, other than minimal
expenses to cover food and transportation, which shall, in any case, be
borne respectively by each party; and
2. It shall be understood that this Agreement does not in any way create an
employer-employee relationship between the Second Party and its
representatives and the interns of the First Party.
C. AMENDMENT
The parties hereto may, with mutual consent, amend or modify this
Agreement through an addendum/amendment duly signed by them any time
before the expiration of this Agreement.

D. EFFECTIVITY
This Agreement shall take effect upon its signing by both parties and shall
remain in force and in effect until ___ days after the last day of the internship,
unless sooner terminated for cause, in which case a written notice to the other
party shall be served at least seven (7) calendar days prior to date of termination.

especially in terms of fire safety operation;
8. Train, expose to, and assist the Interns about the standard operating
procedure of the agency in fire safety and arson investigation.

9. Provide and distribute copies of training materials such as handouts and


other reading materials to the Criminology Interns;
10. Facilitate the coordination between the interns and its security complement
for the duration of the internship;
11. Ensure the personal safety of the interns within the duty premises for the
duration of the internship;
12. Submit a report or reports to CHCC regarding the performance of its interns
through its designated OJT Coordinator; and
13. Accomplish fully the Performance Evaluation Sheets (PES), folded and
sealed, to be provided by the First Party through the interns Team Leader.
