SUBJECT: Spot report re Self-Vehicular Accident.

2. Narrative Report:
At about 6:30 in the evening of June 24,, 2017, a SELF-VEHICULAR ACCIDENT
transpired along the National Highway of Brgy. Sto. Nio, Talakag, Bukidnon involving one (1) unit
of single motorcycle Kawasaki LX150C, color white and green, bearing plate number KA 60782
with engine number LX150CEPD0932 and chassis number MH4LX160CCKP16434, CR number
199669433 dated 11/10/2014 and OR number 555997584 dated 11/10/2014 registered to Fel B.
Bernabe with postal address of Brgy. 5 Poblacion, Talakag, Bukidnon and driven by Jay-R
Dawogan y Salona, male, 18 years old (DOB 10/23/1989), single, farmer and a resident of Brgy. 2
Talakag, Bukidnon without drivers license.

Investigation disclosed that while travelling from west headed towards east direction at
maximum speed and upon reaching at curve lane of place of incident, subject SMC slide towards
the sign post thereby resulted to the incident. Immediately the driver was brought to Bukidnon
Provincial Hospital Talakag for immediate medical treatment, unfortunately he was declared dead
on arrival by his Attending Physician Dr. Lowel Intud.

Further informed that subject SMC is now in custody at this station for proper disposition.
