PATIENT RIGHTS & RESPONSIBILITIES

As a patient, you are afforded impartial access to available and medically
necessary treatment, and accommodations regardless of age, race,
ethnicity, religion, culture, language, physical or mental disability, socio-
economic status, sex, sexual orientation, and gender orientation or
expression. Presence Our Lady of the Resurrection Medical Center
subscribes to the following Patient Rights and Responsibilities that reflect
our Mission and Values as an institution promoting Gods healing presence.
As a patient, you have a right to:
Receive information in a manner tailored to your age, language and
ability to understand.
Exercise your cultural values and spiritual beliefs as long as they do not
interfere with the well-being of others or the planned course of any
medical care.
Effective Communication.
Privacy and confidentiality, including the confidentiality of your clinical
records and the right to access information contained in your clinical
records within a reasonable time frame.
Have pain treated as efficiently as possible.
Access, request amendment to, and obtain information on disclosure of
your health information, in accordance with law and regulations.
Have a family member, friend, or other individual present with you for
emotional support, unless the individuals presence is medically or
therapeutically contraindicated or infringes upon others rights or safety.
Receive medical treatment or accommodations regardless of age, race,
ethnicity, religion, culture, language, physical or mental disability, socio-
economic status, sex, sexual orientation, and gender orientation, or
expression.
Be involved in making informed decisions about your care, treatment
and services, including the right to have your own physician promptly
notified of your admission to the hospital.
Refuse care, treatment and services, in accordance with law and
regulation.
Consent or refuse treatment after being adequately informed of the
benefits and risks, and alternatives to treatment.
Receive information about outcomes of care and any unanticipated
outcomes of care, treatment and services that relate to sentinel events.
Have a surrogate decision-maker make informed decisions about your
care, treatment and services, including the refusal of care, treatment and
services on your behalf, and receive information about unanticipated
outcomes of care, treatment and services, that relate to sentinel events,
when you are unable to make informed decisions.
Give or withhold informed consent to produce or use recording, film or
other images of you for purposes other than your care.
Be fully informed of, and to consent to or refuse to participate in, any
unusual experiment or research project without compromising your
access to service.
Receive information about the individual(s) responsible for, as well as
those providing your care, treatment and services.
Formulate or review and revise your advanced directives and have
members of the hospital staff comply with those advanced directives, in
accordance with law and regulation and the hospitals capabilities.
Have your wishes honored regarding organ donation.
Be free from neglect; exploitation; and verbal, mental, physical and
sexual abuse.
An environment that preserves your dignity and contributes to your
positive self-image.

discrimination, reprisal, or unreasonable interruption of care.
Access protective and advocacy services.
Be free from restraint or seclusion of any form, imposed as a means of
coercion, discipline, convenience, or retaliation by staff (restraint or
seclusion may only be imposed to ensure the immediate physical safety
of the patient, a staff member, or others and must be discontinued at the
earliest possible time).
Receive the visitors whom you designate, subject to your consent,
including, but not limited to, a spouse, a domestic partner (including a
same-sex domestic partner), another family member, or a friend, and to
withdraw or deny such consent at any time. The hospital will not restrict,
limit, or otherwise deny visitation privileges on the basis of age, race,
ethnicity, religion, culture, language, physical or mental disability, socio-
economic status, sex, sexual orientation, and gender orientation, or
expression.
Examine and receive an explanation of your bill, regardless of the source
of payment.
Request an ethics consultation.
As a patient, you have the responsibility to:
Provide, to the best of your knowledge, accurate and complete
information that facilitates your care, treatment and services. This may
include information regarding the following: the nature of your illnesses;
past illnesses and hospitalizations; specific problems, symptoms, pains or
issues you have during your hospital stay; medication history, reactions
and concerns; any ongoing changes in your medical condition as they
occur; and any advance directives you have prepared.
Ask questions or acknowledge when you do not understand the
treatment course or care decision.
Follow instructions, policies, rules and regulations in place to support
quality care for you and a safe environment for all individuals in the
hospital.
Support mutual consideration and respect by maintaining civil language
and conduct in interactions with staff, and licensed independent
practitioners.

issues you may have. A written concern about patient safety, quality of
