MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This MEMORANDUM OF AGREEMENT made and entered into in Bayombong, Nueva

Vizcaya, Philippines, on this __________ day of October 2016 by and between.

PLT COLLEGE, INC.

A non-stock, non-profit educational institution with business address at Bayombong, Nueva

Vizcaya, duly recognized and authorized Higher Educational Institution by the Commission on
Higher Education (CHED), and Technical Education and Skills Development Authority (TESDA)
represented by ENGR. PURISIMO L. TIAM, President, hereafter referred to as the PARTY OF
THE FIRST PART, and

VETRANS REGIONAL HOSPITAL

A government/ devolved hospital of Bayombong, Nueva Vizcaya, represented by CIRILO

R.GALINDEZ, Medical Center Chief II, hereinafter referred to as PARTY OF THE SECOND PART.

WITNESSETH

1. That the students of the PARTY OF THE FIRST PART need to have their practicum to
equip them with adequate working knowledge in the actual office environment which
will prepare them to become efficient and effective individual in their chosen fields;
2. That the PARTY OF THE SECOND PART agrees to, and allows, the pre-service training
of the said students in the Municipality of Solano, Nueva Vizcaya;
3. That the PARTY OF THE FIRST PART AND THE SECOND PART agrees to the following
terms and conditions:

A. The PARTY OF THE FIRST PART shall:

1. supervise student trainees and regularly monitor attendance and performance

during the practicum period through its practicum advise/coordinator;
2. require student trainees to wear prescribed school/OJT uniform and ID at all
times during their hours of duty;
3. provide student evaluation kit to site supervisor which consist of Evaluation for
Trainees Performance.
4. not hold PARTY OF THE SECOND PART responsible for any untoward accident
that occurs beyond its control within its premises during the training period.
5. require daily report of activities from student-trainees.

B. The PARTY OF THE SECOND PART shall:

1. endeavor to assign student trainees in areas related to their field of

specialization wherever possible;
2. provide PARTY OF THE FIRST PART feedback on the students performances
through its site supervisor;
3. adopt measures to provide security and protection of student trainees during
the training period;
4. provide official identification card to student-trainees;
5. only accommodate student-trainees within its absorptive capacity;
6. evaluate performance of trainees.

C. That in the case of inconsistencies, the On-the-Job Training Program policies

and guidelines of PARTY OF THE SECOND PART shall prevail;

The term of this contract is from November ______, 2016 to November _____, 2018,
subject
to renewal after two (2) years.

In witness hereof the parties hereto sign this Memorandum of Agreement this ________
day of _______________________ at Bayombong, Nueva Vizcaya, Philippines.
