Visa Application Form ALL Paper size must be an A4 Bond paper and no erasures. 3. Photo Specs: 4.5cm x 4.5cm with white background. Photo must be pasted on the application form. Taken at least within three (3) months in collared shirt and without eye glasses. 4. Birth Certificate of applicant- (Required issued within one (1) year or else it would not be accepted.) 5. Marriage Contract (if the applicant is married) (4 and 5 must be from NSO Main Office/Serbilis Outlet Center (Nationwide) issued within one (1) year or else it would not be accepted.) EXEMPTION OF BIRTH CERTIFICATES & MARRIAGE CONTRACT If the applicant is has his/her passport (old or valid) with a used Japanese visa. ADDITIONAL DOCUMENTS If Birth Certificate from NSO is UNREADABLE, or If there is no record in NSO, (in addition to "Certificate of Non-Record" from NSO Headquarter) - SUBMIT: Birth certificate from Local Civil Registrar If Birth Certificate (NSO) is "Late Registration" - Baptismal Certificate & School Record (Form 137) - School Yearbook (If applicable) 6. Daily Schedule in Japan (TAIZAI NITTEIHYO) Schedule of Stay (English) must be printed in A4 size. 7. Personal Bank Certificate At least three (3) months valid from the date of issuance. 8. Income Tax Return (Form 2316/1700/1701 etc.) MOST RECENT Employer's Certificate of Compensation Payment/Tax Withheld For Compensation Payment With or Without Tax Withheld and must be submitted in photocopy, A4 size. Must: If a person doesnt have an ITR EXPLANATION LETTER will do. 9. Authorization Letter (Authorizing Travelpros to file & claim the visa application). Must be printed in A4 size.
